摘要
目的遗传因素在未足月胎膜早破(PPROM)发生中起关键作用。文中分析初产妇PPROM与基质金属蛋白酶(MMP)-7、2′,5′-寡腺苷酸合成酶(OAS)1基因多态性的关系。方法回顾性分析2018年5月-2019年7月成都中医药大学附属医院收治的82例PPROM初产妇临床资料(PPROM组);另按1∶1比例选取同期医院分娩的年龄相仿的82例足月妊娠后分娩的初产妇作为对照组。采用聚合酶链反应(PCR)-限制性片段长度多态性(RELP)法测定MMP-7、OAS 1基因型及等位基因分布;酶联免疫吸附试验法测定两组血清MMP-7表达;放射免疫法测定OAS 1表达。分析MMP-7、OAS 1基因多态性与PPROM的关系。结果PPROM组携带AG型(51.22%)基因型所占比例显著高于对照组(36.59%),差异有统计学意义(P<0.05);携带G型等位基因比例(36.59%)显著高于对照组(21.95%),差异有统计学意义(P<0.05)。PPROM组、对照组OAS 1基因rs10774671位点基因型及等位基因频率分布比较差异无统计学意义(P>0.05);PPROM组脐血MMP-7浓度高于对照组(P<0.05),两组OAS1浓度比较差异无统计学意义(P>0.05)。携带MMP-7 rs1156818位点AG基因型可能使PPROM发生风险增加2.128倍(校正OR=2.128,95%CI:1.623~2.788),携带等位基因G可能使PPROM发生风险增加2.149倍(校正OR=2.149,95%CI:1.719~2.387)。结论携带MMP-7(rs1156818位点)等位基因G可能与初产妇PPROM易感性有关;而OAS1 rs10774671位点基因多态性与PPROM无关。
Objective Genetic factors play a vital role in preterm premature rupture of membranes(PPROM).This study sought to investigate the association of PPROM with polymorphisms of matrix metalloproteinase-7(MMP-7)and 2',5'-oligoadenylate synthetase-1(OAS1)genes in primiparas.Methods A total of 82 PPROM primiparas who were admitted to the hospital from May 2018 to July 2019 were enrolled as the PPROM group,while another 82 full-term primiparas with the same age who underwent delivery in the hospital during the same period were enrolled as the control group.The distributions of MMP-7 and OAS1 genotypes and alleles were detected by polymerase chain reaction(PCR)-restriction fragment length polymophism(RELP).The expression of serum MMP-7 in both groups was detected by enzyme linked immunosorbent assay.The expression of OAS1 was detected by radioimmunoassay.The relationship between polymorphisms of MMP-7 and OAS1 genes and PPROM was analyzed.Results A significant higher ratio of AG genotype was found in the PPROM group compared to the control group(51.22%vs 36.59%)(P<0.05),and higher G allele was also observed in the PPROM group(36.59%vs 21.95%)(P<0.05).No significant differences were observed between the PPROM and control group in the frequencies of genotypes and alleles of OAS1 rs10774671(P>0.05).The concentration of cord blood MMP-7 in PPROM group was higher than that in control group(P<0.05),and the difference in OAS1 concentration between the two groups was not statistically significant(P>0.05).AG genotype with MMP-7 rs1156818 might increase the occurrence of PPROM by 2.128 times(revised OR=2.128,95%CI=1.623-2.788),and allele G might increase the occurrence of PPROM by 2.149 times(revised OR=2.149,95%CI=1.719-2.387).Conclusion Carrying MMP-7(rs1156818)allele G may be associated with the susceptibility to PPROM in primiparas,but OAS1 gene polymorphism at rs10774671 is not related to PPROM.
