摘要
目的:探讨胰岛素样生长因子-1(IGF-1)及其受体(IGF-1R)在前列腺癌(PCa)组织中的表达及其临床意义。方法:取2017年1月至2019年12月河北医科大学第三医院泌尿外科的41例前列腺癌患者的前列腺组织作为实验组;并收集21例良性前列腺增生(BPH)患者前列腺组织当做对照组;并通过前列腺特异性抗原(PSA)数值大小、Gleason评分、患者有无远处转移对实验组再次进行分组。利用免疫组织化学法来测定两组前列腺组织中IGF-1、IGF-1R蛋白的表达。用SPSS 22.0统计软件进行数据处理,用χ^(2)检验进行两样本阳性率的对比。结果:IGF-1在对照组和实验组的组织中阳性表达率分别为38.10%、69.44%(χ^(2)=5.347,P<0.05),差异有统计学意义;癌症组织中IGF-1在PSA≤20 ng/ml和PSA>20 ng/ml组别的阳性表达率分别为33.33%、81.48%(P<0.05),差异有统计学意义,在Gleason评分≤7分和Gleason评分>7分组别为58.33%、91.67%(P<0.05),差异有统计学意义,在有远处转移和无转移组别为93.75%、50.00%(P<0.01),差异有统计学意义。IGF-1R在癌症与增生组的表达差异无统计学意义;IGF-1R在Gleason评分≤7分中和Gleason评分>7分组别中的强阳性表达率分别为32%、75%(P<0.05),差异有统计学意义,在不同PSA及有无远处转移分组中的表达差异无统计学意义。结论:IGF-1与前列腺癌的发生及进展有关,而IGF-1R与前列腺癌的恶性程度有关。
Objective To explore the expression and clinical significance of insulin-like growth factor(IGF)-1 and its receptor(IGF-1R)in prostate cancer(PCa)tissues.Methods The pathological tissues of 41 patients with PCa were taken as the experimental group.A total of 21 cases of benign prostatic hyperplasia(BPH)tissues were collected as the control group.The experimental group was grouped again according to the value of prostate specific antigen(PSA),Gleason score,and whether the patients had distant metastasis.The expression of IGF-1 and IGF-1R protein in the prostate tissues of the two groups was determined by immunohistochemistry.The SPSS22.0 statistical software was used for data processing and chi-square test was used to compare the positive rate of two samples.P<0.05 means that the two samples are considered to be statistically significant.Results The positive expression rate of IGF-1 in the control and experimental groups was 38.10%and 69.44%respectively(χ^(2)=5.347,P<0.05).The positive expression rate of IGF-1 in cancer tissues in the PSA≤20 ng/ml and PSA>20 ng/ml groups was 33.33%and 81.48%(P<0.05),58.33%and 91.67%in the Gleason score≤7 and Gleason score>7 groups(P<0.05),93.75%and 50.00%in the distant metastasis and non-metastasis groups,respectively.There was no statistically significant difference in the expression of IGF-1R between cancer and hyperplasia groups.The strong positive expression rate of IGF-1R in the Gleason score≤7 and Gleason score>7 groups was 32%and 75%(P<0.05).There was no statistically significant difference in the expression between PSA groups,and between distant metastasis and non-metastasis groups.Conclusion The IGF-1 signaling system may be related to the occurrence and progression of PCa,and IGF-1R may be related to the malignant degree of prostate cancer.
