摘要
目的:探究Th17细胞与调节性T细胞(Treg)在糜烂性口腔黏膜疾病患者的表达水平及其对黏膜病损发展的意义。方法:取2017年1月~2019年2月我院收治的300例口腔扁平苔藓(OLP)患者(其中糜烂型124例,非糜烂型176例),另选150例健康者作为对照。采用流式细胞法检测各组外周血Th17细胞和Treg细胞水平,采用ELISA检测外周血IL-17、TGF-β1水平,采用RT-PCR法检测各组外周血单个核细胞中RORc mRNA、Foxp3 mRNA水平。结果:OLP患者外周血Th17细胞、Treg细胞、IL-17水平均高于对照组,RORc mRNA、Foxp3mRNA相对表达量也高于对照组,差异具有统计学意义;糜烂型OLP患者RORc mRNA/Foxp3 mRNA比值高于对照组,但非糜烂型OLP患者RORc mRNA/Foxp3 mRNA比值与对照组无显著差异;Spearman相关性分析显示,RORc mRNA与Foxp3mRNA呈正相关,RORc mRNA与体征、RAE评分呈正相关,RORc mRNA/Foxp3 mRNA也与体征、RAE评分均呈正相关。结论:OLP患者外周血中Th17和Treg细胞均处于高水平,但在非糜烂型OLP中Th17/Treg保持平衡,而在糜烂型OLP中存在以Th17为优势的Th17/Treg失衡,推测该失衡在OLP病损形成中起到了重要作用。
Objective To investigate the expression levels of Th17 cells and regulatory T cells (Treg) in patients with erosive oral mucosal disease and its significance in the development of mucosal lesions.Methods From January 2017 to February 2019,300 patients with oral lichen planus (OLP) treated in our hospital (including 124 erosive cases and 176 non-erosive cases) were enrolled,andanother 150 healthy subjects were used as controls.The levels of Th17 cells and Treg cells in peripheral blood of each group were detected by flow cytometry.The levels of IL-17 and TGF-β1 in peripheral blood were detected by ELISA.The RORcmRNA and Foxp3 mRNA levels in peripheral blood mononuclear cells were detected by RT-PCR.Results The levels of Th17 cells,Treg cells and IL-17 in peripheral blood of patients with OLP were higher than those in the control group,andthe relative expression of RORc mRNAand Foxp3 mRNA was also higher than that of the control group.The RORc mRNA/Foxp3 mRNAratio in patients with erosive OLP was higher than that in the control group,but the RORc mRNA/Foxp3 mRNAratio in non-erosive OLP patients was not significantly different from that in the control group.Spearman correlation analysis showed that RORc mRNA was positively correlated with Foxp3 mRNA.The ratio of RORc mRNA and RORc mRNA/Foxp3 mRNA was positively correlated with physical signs and RAE scores.Conclusion Th17 and Treg cells in peripheral blood of OLP patients are at a high level,but Th17/Treg is balanced in non-erosive OLP,while Th17/Treg imbalance in Th17 is dominant in erosion OLP,whichcan be speculated that the imbalanceplays an important role in the formation of OLP lesions..
