C反应蛋白乳酸脱氢酶水平检测在重症肺炎支原体肺炎早期识别中的应用 被引量:9

摘要 目的分析重症肺炎支原体肺炎(MPP)患儿白细胞(WBC)、C反应蛋白(CRP)、乳酸脱氢酶(LDH)、红细胞沉降率(ESR)水平,探究其是否可以作为重症MPP的早期识别标志。方法选取2018年1月至2019年12月山西省儿童医院(山西省妇幼保健院)收治的100例重症MPP患儿为观察组,100例普通MPP患儿为对照组进行回顾性研究;检测血中WBC、CRP、LDH及ESR水平,绘制ROC曲线分析各因素在重症MPP中的预测价值。结果观察组血中CRP、LDH水平明显高于对照组,且差异有统计学意义(P<0.05);LDH>343 U/L对重症MPP具有最佳的预测价值。结论 CRP、LDH升高,且LDH>343 U/L可以作为重症MPP早期识别的检测指标,LDH>343 U/L常提示患儿为重症病例,需要及时调整治疗方案。 Objective To determine the levels of white blood cell(WBC),C reactive protein(CRP),lactate dehydrogenase(LDH)and erythrocyte sedimentation rate(ESR)in children with severe mycoplasma pneumoniae pneumonia(MPP),and to investigate whether they can be used as indicators for early identification of severe MPP.Methods A total of 100 children with severe MPP admitted to Shanxi Children′s Hospital(Shanxi Provincial Maternal and Child Health Hospital)between January 2018 and December 2019 were included as the study group,and 100 children with ordinary MPP were included as the control group for the retrospective study.The blood WBC,CRP,LDH and ESR levels were examined,and the ROC curve was drawn to analyze the predictive value of each indicator in severe MPP.Results The levels of CRP and LDH in the study group were significantly higher than those in the control group,with statistically significant differences(P<0.05).LDH>343 U/L showed the best predictive value for severe MPP.Conclusion The elevated CRP and LDH levels,and LDH>343 U/L may be used as indicators for early identification of severe MPP.LDH>343 U/L is commonly used for severe cases,and the treatment protocol requires timely adjustment.
