上海地区老年人群血常规参数的参考值范围调查

Investigation on reference range of blood routine among elderly population in Shanghai

目的确立上海地区60岁以上老年人群血常规各血细胞参数的参考值区间。方法采集2018年7月2日至9月3日在华东医院进行健康体检的60岁以上老年人群,以全自动血细胞分析仪检测血常规指标。结果健康人群的血常规各参数结果呈非正态分布。不同性别的血常规水平差异有统计学意义,男性的红细胞相关指标[红细胞计数(RBC)、血红蛋白浓度(HGB)、红细胞压积(HCT)、平均红细胞血红蛋白量(MCH)、平均红细胞血红蛋白浓度(MCHC)]、白细胞相关指标[白细胞计数(WBC)、中性粒细胞(NEUT)、单核细胞计数(MONO)、嗜酸性粒细胞(EO)和嗜碱性粒细胞计数(BASO)]较高,但女性血小板计数(PLT)和淋巴细胞(LYMPH)水平较高。不同年龄(60~69岁、70~79岁以及≥80岁,除MCHC、WBC、NEUT、BASO和BASO%外)的血常规水平差异有统计学意义,RBC、HCT、HGB、PLT、LYMPH和LYMPH%水平均有随年龄增加而减少的趋势;而MCV、MCH、MONO和MONO%、EO和EO%水平则有随年龄增加而增加的趋势。结论老年人的血常规参考值范围与成年人有异,并且年龄和性别影响血常规参考值范围,建立符合本地区老年人群的血常规参考值范围至关重要。

Objective To establish the reference range of blood cell parameters in blood routine of the elderly population over 60 years old in Shanghai.Methods The elderly subjects aged over 60 years old,who underwent physical examination at Huadong Hospital between July 2nd and September 3rd,2018 were included in the study.The blood routine parameters were examined by automatic blood cell analyzer.Results The results of blood routine parameters of the healthy subjects showed non-normal distribution.There were statistically significant differences in the blood routine levels between different genders.The reference ranges of red blood cell-related indexes[red blood cell(RBC),hemoglobin(HGB),hematocrit(HCT),mean corpuscular hemoglobin(MCH),and mean corpuscular hemoglobin concentration(MCHC)]and white blood cell-related indexes[white blood cell(WBC),neutrophils(NEUT),monocyte(MONO),eosinophils(EO)and basophils(BASO)]were relatively high in males,whereas the platelet(PLT)count and lymphocyte(LYMPH)level were relatively high in females.There were statistically significant differences in the blood routine levels(except for MCHC,WBC,NEUT,BASO and BASO%)among the subjects with different ages(60-69 years old,70-79 years old and≥80 years old).The levels of RBC,HCT,HGB,PLT,LYMPH and LYMPH%showed a decreasing trend with the increase of age.However,the levels of MCV,MCH,MONO and MONO%,EO and EO%showed tended to increase with age.Conclusion The reference range of blood routine for the elderly is different from that for adults,and age and gender are influence factors for the reference range of blood routine.It is very important to establish a reference range of blood routine based on the elderly population in this region.
