百乐眠胶囊联合重复经颅磁刺激治疗缺血性脑卒中继发失眠的临床疗效及其对多导睡眠图参数的影响

Clinical effect and impact on polysomnography parameters of bailemian capsule combined with repetitive transcranial magnetic stimulation on insomnia secondary to ischemic stroke

目的探讨百乐眠胶囊联合重复经颅磁刺激(rTMS)治疗缺血性脑卒中继发失眠的临床疗效及其对多导睡眠图(PSG)参数的影响。方法选取常州市德安医院康复中心2019年3月—2020年6月收治的缺血性脑卒中伴继发性失眠患者50例,采用随机数字表法分为对照组与观察组,每组25例。对照组给予百乐眠胶囊治疗,观察组给予百乐眠胶囊联合rTMS治疗。比较2组治疗前后睡眠质量参数[包括睡眠潜伏期(SL)、觉醒的时间(AWT)、睡眠的效率(SE)、觉醒的次数(AT)、总的睡眠时间(TST)]、美国国立卫生院卒中量表(NIHSS)评分、医院焦虑抑郁量表(HADS)评分[包括焦虑情绪(HADS-A)评分与抑郁情绪(HADS-D)评分],并观察2组不良反应发生情况。结果治疗前2组SL、AWT、SE、AT、TST比较,差异无统计学意义(P>0.05);治疗后观察组SL、AWT短于对照组,SE高于对照组,AT少于对照组,TST长于对照组(P<0.05)。治疗前2组NIHSS评分、HADS-A评分、HADS-D评分比较,差异无统计学意义(P>0.05);治疗后观察组NIHSS评分、HADS-A评分、HADS-D评分低于对照组(P<0.05)。2组治疗期间均未发生明显不良反应。结论百乐眠胶囊联合rTMS治疗缺血性脑卒中继发失眠的临床疗效确切,可有效改善患者睡眠质量,促进神经功能恢复及缓解负性情绪,且安全性较高。

Objective To investigate the clinical effect and impact on polysomnography parameters of bailemian capsule combined with repetitive transcranial magnetic stimulation(rTMS)on insomnia secondary to ischemic stroke.Methods A total of 50 patients with insomnia secondary to ischemic stroke were selected from March 2019 to June 2020 in Changzhou Dean Hospital Recovery Center,which were randomly divided into control group and observation group,25 cases in each group.The control group was treated with bailemian capsule,while the observation group was treated with bailemian capsule combined with rTMS.Sleep quality parameters(including SL,AWT,SE,AT and TST),NIHSS score and HADS score(including HADS-A and HADS-D)before and after treatment were compared between the two groups,and the incidence of adverse reactions were observed.Results There was no significant difference in SL,AWT,SE,AT and TST between the two groups before treatment(P>0.05);After treatment,the observation group of SL and AWT were shorter than control group,SE was higher than control group,AT was less than control group and TST was longer than control group(P<0.05).There was no significant difference in NIHSS score,HADS-A score and HADS-D score between the two groups before treatment(P>0.05);after treatment,NIHSS score,HADS-A score and HADS-D score in observation group were lower than control group(P<0.05).No obvious adverse reactions occurred in the two groups during treatment.Conclusion Bailemian capsule combined with rTMS have an exact clinical effect for insomnia secondary to ischemic stroke,which can effectively improve the sleep quality of patients,promote the recovery of neurological function and relieve negative emotions,and with high safety.
