口腔扁平苔藓患者情绪障碍与唾液中谷氨酰胺含量的相关性

Correlation between emotional disorders and glutamine levels in saliva of patients with oral lichen planus

目的采用情绪障碍自评量表和代谢组学的研究方法,评估口腔扁平苔藓(oral lichen planus,OLP)患者情绪障碍和唾液中谷氨酰胺含量的相关性。方法选取2019年3—8月于哈尔滨医科大学附属第一医院初次被确诊为OLP的患者60例,分为糜烂型OLP组(30例)、非糜烂型OLP组(30例),另选择同期于本院进行口腔检查的30例健康人群作为对照组,采用广泛性焦虑障碍量表、抑郁自评量表调查问卷对OLP患者和对照组进行情绪障碍评估,运用液相色谱-质谱联用技术对三组的唾液样本进行代谢组学分析,探讨情绪障碍和唾液中谷氨酰胺含量的相关性。结果与对照组相比,OLP患者广泛性焦虑障碍量表、抑郁自评量表得分明显升高,唾液中谷氨酰胺含量明显降低,差异有显著性(P<0.05)。糜烂型OLP组患者的情绪障碍得分最高,唾液中谷氨酰胺含量最低,三组比较差异有显著性(P<0.05)。Pearson相关分析显示GAD-7、PHQ-9得分与唾液中谷氨酰胺含量均呈负相关。结论OLP患者情绪障碍得分与唾液中谷氨酰胺含量呈负相关,提示在口腔扁平苔藓的治疗中,除了心理干预,外源性补充谷氨酰胺可能对疾病的缓解具有潜在作用。

Objective To evaluate the correlation between emotional disorders and glutamine levels in saliva of patients with oral lichen planus with the help of the self-evaluation scale for mood disorders and metabolomics.Method With the tool of the GAD-7 and the PHQ-9 questionnaire,emotional disorders were evaluated in patients with oral lichen planus and healthy controls.A mass spectrometry(MS)technique was used to perform metabolomics analysis of saliva samples from patients with erosive oral lichen planus,non-erosive oral lichen planus and healthy controls to find out the correlation between mood disorders and glutamine content in saliva.Result Compared with the healthy patients of control group,patients with oral lichen planus had significantly higher GAD-7 and PHQ-9 scores and significantly lower glutamine in saliva.Patients with erosive oral lichen planus had the highest scores for mood disorders and the lowest glutamine content in saliva.Conclusion Elevated mood disorder scores of patients with oral lichen planus are significantly correlated with decreased glutamine content in saliva.It is suggested that in the treatment of patients with oral lichen planus,in addition to psychological intervention,exogenous glutamine supplementation may have a potential effect on the relief of the disease.
