摘要
目的探讨云端管理系统对慢性肾脏病患者生化指标及不良肾脏结局的影响。方法选取2018年1-12月于该中心肾内科住院的未行肾脏替代治疗的慢性肾脏病4~5期患者为研究对象,分为研究组98例,对照组94例。对照组采取传统管理模式,研究组采取云端管理模式。对比两组患者1年后生化指标达标率、不良肾脏结局发生率、健康知识掌握及生活方式情况。结果两组患者全因死亡比较无差异(P>0.05),研究组患者不良肾脏结局发生率明显低于对照组(P<0.05)。研究组患者血钙、血磷、ipth、清蛋白、血压、身体质量指数达标率明显高于对照组(P<0.05);对高磷食物品种、去磷烹饪技巧、高蛋白低磷食物品种、低嘌呤食物品种、高脂饮食种类的掌握情况均明显高于对照组(P<0.05);吸烟率、饮酒率明显低于对照组,运动率明显高于对照组(P<0.05)。两组患者三酰甘油水平、血红蛋白达标率比较无差异(P>0.05)。结论云端管理模式能降低慢性肾脏病4~5期患者不良肾脏结局发生率,提高临床生化指标的达标率、健康知识掌握率及健康生活方式的采用率。
Objective To explore the impact of cloud management system on the biochemical indicators and adverse renal outcomes in the patients with chronic kidney disease(CKD).Methods The inpatients with the stage 4-5 of CKD without conducting the renal replacement therapy admitted to the nephrology department of this hospital from January 2018 to December 2018 were selected as the research subjects and divided into 98 cases in the study group and 94 cases in the control group.The control group adopted the traditional management mode,and the study group adopted the cloud management mode.After one year,the rate of biochemical indicators reaching the standard,incidence rate of adverse kidney outcomes,grasping the health knowledge and lifestyles were compared between the two groups.Results There was no difference in all-cause mortality between the two groups(P>0.05).The incidence arte of adverse renal outcomes in the study group was significantly lower than that in the control group(P<0.05);the level of blood calcium,blood phosphorus,ipth,albumin,blood pressure,and reaching standard rate of body mass index in the study group were significantly higher than those in the control group(P<0.05);the grasping situation of high phosphorus food varieties,dephosphorization cooking techniques,and high protein low phosphorus food varieties,kinds of low-purine foods and high-fat diets in the study group were significantly higher than those in the control group(P<0.05);the smoking and drinking rates in the study group were significantly lower than those in the control group,and the exercise rate was significantly higher than that in the control group(P<0.05);there was no difference in triacylglycerol level and hemoglobin reaching standard rate between the two groups(P>0.05).Conclusion The cloud management model can reduce the incidence rate of adverse kidney outcomes in the patients with the stage 4-5 of CKD,increase the reaching standard rates of clinical biochemical indicators,grasping rate of health knowledge and the adopting rate of healthy lifestyles.
