固本通络汤联合激素及环磷酰胺治疗难治性肾病综合征78例

Treating 78 cases of refractory nephrotic syndrome with the Guben Tongluo decoction plus hormone and cyclophosphamide

目的:探究固本通络汤联合激素及环磷酰胺治疗难治性肾病综合征(Refractory Nephotic Syndrome,RNS)的临床效果。方法:选取2017年9月-2019年12月于天门市中医医院接受治疗的156例RNS患者,按随机数字表法分为两组,各78例。对照组行激素及环磷酰胺治疗,观察组在对照组基础上联合固本通络方治疗。对比两组临床疗效、治疗前后中医症状积分、肾功能各指标水平[血尿素氮(Blood Ure Nitrogen,BUN)、血清肌酐(Serum Creatinine,Scr)、血清白蛋白(Albumin,ALB)及24h尿蛋白定量(24-hour Urinary Protein Quantity,24 h UP)]、不良反应。结果:相比于对照组,观察组总有效率高、治疗后中医证候积分较低,差异有统计学意义(P<0.05);观察组治疗后BUN、Scr、24 h UP均低于对照组,ALB含量高于对照组,差异有统计学意义(P<0.05);观察组不良反应少于对照组,差异有统计学意义(P<0.05)。结论:固本通络汤联合激素及环磷酰胺在RNS治疗中疗效确切,可有效降低中医症状积分,改善肾功能各指标水平,利于促进预后恢复,值得推广应用。

Objective:To explore the clinical effects of the Guben Tongluo decoction(固本通络汤)plus hormones and cyclophosphamide on refractory nephotic syndrome.Methods:156 RNS patients who were treated in our hospital from September 2017 to December 2019 were selected and divided into two groups according to the random number table method,with 78 cases in each group.The control group was treated with hormones and cyclophosphamide,and the observation group was treated with the Guben Tongluo decoction more.Results:Compared with the control group,the total efficiency in the observation group was higher,and TCM syndrome scores after treatment were lower(P<0.05).After treatment,BUN,Scr and 24 h UP content in the observation group was lower than the control group,and ALB content was higher than the control group,P<0.05.The adverse reactions in the observation group were fewer than the control group,P<0.05.Conclusion:The Guben Tongluo decoction plus hormones and cyclophosphamide shows a definite effect on RNS,can effectively reduce the TCM syndrome scores,ameliorate the levels of various indicators of renal function,promote the recovery of prognosis,and is worthy of popularization and application.
