越婢加半夏汤对慢性阻塞性肺病急性发作(痰热郁肺型)患者的临床疗效观察

Clinical observation on treating AECOPD of the Tanre Yufei type with the Yuebi plus Banxia decoction

目的:探讨越婢加半夏汤对慢性阻塞性肺病急性发作期(Acute Exacerbation of Chronic Obstructive Pulmonary Disease,AECOPD)患者血清纤维结合蛋白(Fn)的影响。方法:本研究选取辨证属痰热郁肺型慢性阻塞性肺病急性发作患者,共120例,随机分为治疗组60例和对照组60例。治疗组进行西医常规处理+越婢加半夏汤,对照组进行西医常规处理,观察并比较两组患者治疗前后血清Fn。两组患者均治疗后观察并比较临床疗效以及中医症状积分、肺功能指标改变况。结果:治疗前两组Fn水平无明显差异;经过治疗后,两组患者的血清Fn都较治疗前提高,差异有统计学意义(P<0.05)。治疗后,治疗组的血清Fn水平高于对照组,差异有统计学意义(P<0.05)。两组患者主要中医证候评分治疗前差异无统计学意义。治疗后,治疗组优于对照组,差异有统计学差异(P<0.05)。治疗组显效率和总有效率均明显高于对照组(P<0.05)。两组患者治疗后肺功能指标较治疗前明显改善(P<0.05),治疗后组间比较差异有统计学意义(P<0.05)。结论:在常规治疗基础上加用越婢加半夏汤治疗AECOPD痰热郁肺证可能通过提升血清Fn水平,明显改善临床症状体征和肺功能,提高疗效。值得临床推广。

Objective:To investigate the effect of the Yuebi plus Banxia decoction(越婢加半夏汤)on serum FN in patients with AECOPD.Methods:120 patients were randomly divided into the treatment group(60 cases)and control group(60 cases).The treatment group was treated with routine Western medicine and the Yuebi plus Banxia decoction,while the control group was treated with routine Western medicine.Results:After treatment,the serum Fn levels in two groups were higher than before treatment,with statistically significant differences(P<0.05),and the serum Fn level in the treatment group was higher than the control group(P<0.05).After treatment,The main TCM syndrome scores in the treatment group was better than the control group,the difference was statistically significant(P<0.05).The effective rate and total effective rate in the treatment group were significantly higher than the control group(P<0.05).The pulmonary function indexes in two groups were significantly improved after treatment(P<0.05),and there was statistically significant difference between groups after treatment(P<0.05).Conclusion:On the basis of conventional treatment,the Yuebi plus Banxia decoction can significantly improve the clinical symptoms,lung function and curative effect by increasing the serum FN level,is worthy of clinical promotion.
