持续性心房颤动射频导管消融术后1年复发因素分析

Analysis of recurrence factors of persistent atrial fibrillation one year after radiofrequency catheter ablation

目的分析持续性心房颤动(简称房颤)射频导管消融术后远期复发的相关危险因素。方法连续入选北部战区总医院自2018年12月至2019年12月收治的129例首次行压力感知导管结合消融指数(AI)指导的射频导管消融术的持续性房颤患者为研究对象。根据是否远期复发,将患者分入远期复发组和未复发组。比较两组患者的临床资料,采用多元logistic回归分析危险因素。结果空白期内复发34例,远期复发39例,远期未复发90例,房颤复发率为30.2%(39/129)。两组患者的总病程、持续时间、术后P波离散度(Pd)及空白期内复发率差异有统计学意义(P<0.05)。总病程、持续时间、术后Pd、空白期内复发与持续性房颤射频消融术后复发独立相关(P<0.05)。结论房颤的持续时间、总病程、术后Pd及空白期内复发是应用压力感知导管结合AI新技术指导的射频导管消融术后1年内复发的独立影响因素。

Objective To analyze the risk factors of long-term recurrence in patients with persistent atrial fibrillation after radiofrequency ablation.Methods 129 patients with persistent atrial fibrillation undergoing radiofrequency catheter ablation under the guidance of new technology of contactforce catheter combined with ablation index(AI)from December 2018 to December 2019 were enrolled.Patients were divided into long-term recurrence group and non-recurrence group.Clinical data of the two groups were compared, and risk factors were analyzed by multiple logistic regression.Results 30 cases recurred in blank period, 90 cases maintained sinus rhythm, 39 cases had long-term recurrence, the recurrence rate of atrial fibrillation was 30.2%(39/129).There were significant differences in the total course of disease, duration, postoperative P wave dispersion and recurrence rate in blank period between 2 groups(P<0.05).Total course of disease, duration, postoperative P wave dispersion and recurrence rate in blank period were independently associated with recurrence after radiofrequency ablation of persistent atrial fibrillation(P<0.05).Conclusion Total course of disease, duration, postoperative P wave dispersion and recurrence rate in blank period are independent risk factors for long-term recurrence of persistent atrial fibrillation after radiofrequency catheter ablation guided by contact force catheter combined with AI.
