球囊后扩张对急性心肌梗死患者直接经皮冠状动脉介入术影响研究

Impact of balloon post-dilation on the prognosis of patients with acute myocardial infarction after primary percutaneous coronary intervention

目的探讨球囊后扩张对急性心肌梗死(AMI)患者直接经皮冠状动脉介入术(PPCI)后不良事件发生率的影响。方法回顾性分析北部战区总医院自2016年3月至2019年3月收治的接受PPCI的2849例AMI患者的临床资料。根据PPCI后是否进行球囊后扩张,将患者分为非后扩张组(n=867)和后扩张组(n=1982)。主要研究终点为随访1年后的主要心脑不良事件(MACCE)的发生率,包括心性死亡、非致死性心肌梗死、卒中。次要研究终点为全因死亡事件发生率。结果后扩张组MACCE、全因死亡、心性死亡发生率低于非后扩张组,差异有统计学意义(P<0.05)。结论球囊后扩张可降低接受PPCI的AMI患者的MACCE发生率,改善患者预后。

Objective To investigate the impact of balloon post-dilation on the prognosis of patients with acute myocardial infarction(AMI)after primary percutaneous coronary intervention(PPCI).Methods The clinical data of 2 849 patients with AMI who received PPCI from March 2016 to March 2019 were retrospectively analyzed.According to whether balloon post-dilation was performed, patients were divided into non-post-balloon dilatation group(n=867)and post-balloon dilatation group(n=1982).The primary endpoint was the incidence of major adverse cardiac and cerebrovascular event(MACCE),including cardiac death, non-fatal myocardial infarction and strokeafter 1 year of follow-up.The secondary endpoint was the incidence of all-cause deaths.Results The incidence of MACCE,all-cause death and cardiac death in post-balloon dilatation group was lower than that in non-post-balloon dilatation group, and the difference was statistically significant(P<0.05).Conclusion Balloon post-dilation can reduce the incidence of MACCE in AMI patients receiving PPCI and improve patient prognosis.
