茎突综合征的锥形束CT研究

The use of cone bean computed tomography in assessing the styloid process syndrome

摘要 目的探讨茎突综合征的发病因素。方法收集30例茎突综合征患者,使用锥形束CT(cone beam computed tomography,CBCT)扫描,测量左右两侧颞骨茎突长度、体积、前倾角、内倾角,与正常人群茎突的各项指标进行对照,研究茎突综合征患者茎突的各项参数指标和特点,探讨茎突综合征与这些指标的关系。结果茎突综合征患者茎突的参数指标:双侧茎突长度分别为50.124±5.161mm和51.353±5.406mm;双侧茎突前倾角分别为29.331±8.027°和28.341±7.469°;双侧茎突内倾角分别为25.141±8.015°和26.528±2.751°;双侧茎突体积分别为426.379±54.863mm^(3)和477.621±92.274mm^(3)。两组人群间双侧茎突长度、内倾角和体积有明显差异(P<0.05),而双侧茎突前倾角无明显差异(P>0.05)。结论 CBCT可清晰显示颞骨茎突影像,用于茎突综合征的影像诊断。茎突综合征与茎突的长度、内倾角和体积的变化有一定关系,而与茎突的前倾角关系不密切。 Objective To investigate the pathogenic factors of the styloid process syndrome(SPS). Methods The length, volume, anteversion angle, and inclination of the bilateral styloid process of temporal bone were measured in 30 patients with SPS and healthy volunteers by cone bean computed tomography(CBCT) to measure,aiming to identify typical anatomic hallmarks associated with SPS. Results The length, volume, anteversion angle,and inclination of the bilateral styloid process of temporal bone in patients with SPS were 50.124 ±5.161 mm and51.353±5.406 mm, 25.141 ±8.015° and 26.528±2.751°, 25.141±8.015° and 26.528±2.751°, and 426.379 ±54.863 mm^(3) and 477.621±92.274 mm^(3) respectively. There were significant differences in the length, volume and inclination of the bilateral styloid process of temporal bone between SPS patients and healthy volunteers(P <0.05). However, no significant difference in the anteversion angle was identified between them(P>0.05). Conclusion CBCT can be used for imaging diagnosis of SPS, which clearly visualizes the structure of the styloid process of temporal bone.SPS is closely linked to the length, volume, anteversion angle, and inclination changes of the bilateral styloid process of temporal bone, while it is not correlated to the anteversion angle of the styloid process.
