在线血液透析滤过联合血液透析在维持性血液透析患者中的应用

Application of online hemodiafiltration combined with hemodialysis in maintenance hemodialysis patients

目的探讨在线血液透析滤过联合血液透析在维持性血液透析患者中的应用效果。方法选取2018年4月至2019年4月本院收治的维持性血液透析患者82例,按照随机数表法分为两组,各41例。对照组接受单纯血液透析治疗,观察组在对照组基础上联合在线血液透析滤过治疗。比较两组血红蛋白(Hb)、红细胞压积(Hct)、血清铁蛋白(SF)、血肌酐(SCr)、尿素(Urea)、C反应蛋白(CRP)水平、β_(2)-微球蛋白(β_(2)-MG)、尿素清除指数(Kt/V)。结果治疗前,两组Hb、Hct、SF水平比较差异无统计学意义;治疗后,观察患者Hb及Hct水平均高于对照组,差异有统计学意义(P<0.05);治疗前,两组SCr、Urea、β_(2)-MG、CRP、Kt/V水平比较差异无统计学意义;治疗后,观察组患者β_(2)-MG、CRP水平均低于对照组,Kt/V水平高于对照组,差异有统计学意义(P<0.05)。结论在线血液透析滤过联合血液透析应用于维持性血液透析患者中,能降低机体的炎症反应、改善营养状态,且有利于纠正肾性贫血。

Objective To explore the application effect of online hemodiafiltration combined with hemodialysis in maintenance hemodialysis patients.Methods 82 patients with maintenance hemodialysis admitted to our hospital from April 2018 to April 2019,and they were divided into two groups,with 41 cases in each group according to the random number table method.The control group was treated with simple hemodialysis,the observation group was treated with online hemodiafiltration on the basis of the control group.The levels of hemoglobin(Hb),hematocrit(Hct),serum ferritin(SF),serum creatinine(SCr),urea(Urea),C-reactive protein(CRP),β_(2)-microglobulin(β_(2)-MG)and urea clearance index(Kt/V)were compared between the two groups.Results Before treatment,there was no significant difference in the levels of Hb,Hct,and SF between the two groups;after treatment,the Hb and Hct levels were higher than those of the control group,the difference was statistically significant(P<0.05);before treatment,there was no significant difference in the levels of SCr,Urea,β_(2)-MG,CRP,and Kt/V between the two groups;after treatment,all theβ_(2)-MG,CRP levels in the observation group were lower than those in the control group,Kt/V levels were higher than those in the control group,the difference was statistically significant(P<0.05).Conclusion Online hemodiafiltration combined with hemodialysis can reduce inflammation and improve nutritional status in maintenance hemodialysis patients,and it is beneficial to correct renal anemia.
