清肝降压胶囊治疗原发性高血压的有效性及对患者血清同型半胱氨酸和血管内皮生长因子水平的影响

Effectiveness of Qinggan Jiangya capsules on treating essential hypertension and its effect on serum homocysteine and vascular endothelial growth factor levels

目的探讨清肝降压胶囊治疗原发性高血压(EH)患者的有效性及对血清同型半胱氨酸(Hcy)、血管内皮生长因子(VEGF)水平的影响。方法采用单臂、开放、多中心试验方法,选取浙江省丽水市人民医院、四川省广安市人民医院2018年1月至2019年1月收治的EH(1~2级)患者392例,均给予清肝降压胶囊治疗。观察392例患者治疗前及治疗2、4周后血压水平、中医证候积分、血清Hcy与VEGF水平、肝肾功能和心率。结果392例患者治疗2、4周后坐位舒张压、收缩压水平[(85±6)、(83±5)mmHg(1 mm Hg=0.133 kPa)比(96±4)mmHg,(150±15)、(147±13)mmHg比(165±10)mmHg]和眩晕、头痛、心悸、心烦急躁、失眠证候积分及血清Hcy、VEGF水平[(15±4)、(13±3)mmol/L比(22±4)mmol/L,(103±28)、(98±12)ng/L比(146±19)ng/L]均低于治疗前,且治疗4周后上述指标水平均低于治疗2周后,差异均有统计学意义(均P<0.05)。392例患者治疗前和治疗2、4周后天冬氨酸转氨酶、丙氨酸转氨酶、白蛋白、总胆红素与血尿素氮、血肌酐、心率水平比较,差异均无统计学意义(均P>0.05)。结论清肝降压胶囊治疗EH患者,可促进患者血压、临床症状及血清Hcy与VEGF水平改善,且对其肝肾功能与心率无明显影响。

Objective To investigate the effectiveness of Qinggan Jiangya capsules on treating essential hypertension(EH)and its effect on serum homocysteine(Hcy)and vascular endothelial growth factor(VEGF)levels.Methods A single arm,open,multicenter trial was performed.Totally 392 patients with EH(grade 1-2)admitted to Lishui City People’s Hospital,Zhejiang Province and Guang’an People’s Hospital,Sichuan Province from January 2018 to January 2019 were enrolled.The patients were treated with Qinggan Jiangya capsules.Blood pressure level,traditional Chinese medicine syndrome score,serum Hcy and VEGF levels,liver and kidney function,and heart rate were observed in 392 patients before and 2,4 weeks after treatment.Results After 2 and 4 weeks of treatment,the sitting diastolic blood pressure and systolic blood pressure[(85±6),(83±5)mm Hg vs(96±4)mm Hg;(150±15),(147±13)mm Hg vs(165±10)mm Hg],the syndrome scores of dizziness,headache,palpitation,irritability,insomnia,and serum Hcy and VEGF levels[(15±4),(13±3)mmol/L vs(22±4)mmol/L;(103±28),(98±12)ng/L vs(146±19)ng/L]were lower than those before treatment,and those at 4 weeks after treatment were lower than those at 2 weeks after treatment(all P<0.05).There were no significant differences in aspartate aminotransferase,alanine aminotransferase,albumin,total bilirubin,blood urea nitrogen,serum creatinine and heart rate in 392 patients before treatment and 2 or 4 weeks after treatment(all P>0.05).Conclusion The application of Qinggan Jiangya capsules in the treatment of patients with EH can promote the improvement of blood pressure,clinical symptoms and serum Hcy and VEGF levels,and it has no significant effect on the liver and kidney functions and heart rate.
