摘要
目的:观察中国汉族妇女外阴鳞状上皮增生患者中医证型与HLA-A\B\DRB1基因多态性的相关性。方法:选取中国汉族妇女外阴鳞状上皮增生中医辨证属肝肾阴虚型36例、肝经湿热型38例,正常中国汉族妇女62例(正常对照组)分别采用PCR-SSP方法进行基因多态性关联检测分析。结果:HLA-B*15等位基因在外阴鳞状上皮增生两组中出现的频率均升高,与正常对照组相比有显著性差异(P<0.01);HLA-B*40等位基因在两组中呈低表达,与正常对照组比较有显著性差异(P<0.01);HLA-DRB1*12等位基因在外阴鳞状上皮肝经湿热型出现频率高,与正常对照组相比有显著性差异(P<0.01);HLA-A*02基因的出现频率在外阴鳞状上皮增生肝肾阴虚型中明显低于正常对照组(P<0.01)。结论:对于中国汉族妇女外阴鳞状上皮增生肝肾阴虚型与肝经湿热型,HLA-B*15等位基因可能是易感基因,HLA-B*40等位基因可能是保护性基因。HLA-DR12等位基因可能是肝经湿热型鳞状上皮增生的易感基因,而与肝肾阴虚型无关;HLA-A*02等位基因可能是外阴鳞状上皮增生肝肾阴虚型的抗性基因。
Objective:To observe the correlation between TCM syndromes and HLA-A\B\DRB1 gene polymorphism in Chinese Han women with vulvar squamous hyperplasia.Methods:36 cases of Chinese Han women with vulvar squamous hyperplasia,36 cases with Yin deficiency of liver and kidney,38 cases with damp-heat of liver channel and 62 cases of normal Chinese Han women(normal control group)were selected for correlation analysis by PCR-SSP method.Results:The frequency of HLA-B*15 allele in vulvar squamous hyperplasia was increased in both groups,and there was a significant difference compared with normal control group(P<0.01).The expression of HLA-B*40 allele in the two groups was significantly lower than that in the normal control group(P<0.01).The frequency of HLA-DRB1*12 allele was higher in damp-heat of liver channel syndrome of vulvar squamous hyperplasia,and the difference was significant compared with normal control group(P<0.01).The occurrence frequency of HLA-A*02 gene in vulvar squamous hyperplasia with Yin deficiency of liver and kidney syndrome was significantly lower than that in normal control group(P<0.01).Conclusion:HLA-B*15 allele may be a susceptibility gene and HLA-B*40 allele may be a protective gene for vulvar squamous hyperplasia of Chinese Han women with Yin deficiency of liver and kidney and damp-heat of liver channel syndromes.The HLA-DR12 allele may be the susceptibility gene for the vulvar squamous hyperplasia of damp-heat of liver channel syndrome,but has nothing to do with the Yin deficiency of liver and kidney.HLA-A*02 allele may be the resistance gene for vulvar squamous hyperplasia of Yin deficiency of liver and kidney syndrome.
