摘要
目的探讨阿尔茨海默病(Alzheimer s disease,AD)患者血清白细胞介素-13(IL-13)、同型半胱氨酸(Hcy)的表达水平及其临床意义。方法根据认知障碍程度,将156例AD患者分为轻度组(95例)和中重度组(61例);另选100名同期健康体检者作为对照组。对比各组一般资料和生化指标的差异,采用Pearson法对AD患者血清IL-13、Hcy水平与疾病相关指标[全面衰退量表(GDS)、简易精神状态检查量表(MMSE)及临床痴呆评定量表(CDR)评分)]进行相关性分析,并绘制ROC曲线分析血清IL-13、Hcy对AD的诊断效能。结果观察组患者叶酸、IL-13水平及MMSE评分低于对照组,血清Hcy水平及GDS、CDR评分显著高于对照组,差异均有统计学意义(P<0.05);2组间其余指标差异均无统计学意义(P>0.05)。中重度组患者Hcy水平及GDS、CDR评分显著高于轻度组,IL-13、叶酸水平及MMSE评分显著低于轻度组,差异有统计学意义(P<0.05)。相关分析结果显示,血清IL-13与叶酸、MMSE评分呈正相关(P<0.05);Hcy水平与叶酸、MMSE评分呈负相关(P<0.05),与GDS、CDR评分呈正相关(P<0.05)。血清IL-13、Hcy的ROC曲线下面积均较大,且P<0.05,对诊断AD均具有较大诊断性能。结论AD患者血清IL-13、Hcy水平与其认知障碍评分及痴呆程度有关,可考虑作为早期诊断疾病的血清学指标。
Objective To explore the level of serum IL-13,homocysteine(Hcy),and the clinical significance of patients with Alzheimer s disease(AD).Methods According to the degree of cognitive impairment,156 AD patients were divided into a mild group(95 cases),a moderate and severe group(61 cases).At the same time,100 healthy individuals comprised the control group.General data and biochemical indexes of each group were compared.Pearson method was used to analyze the correlation between the levels of serum IL-13,Hcy and disease-related indicators such as Global Deterioration Scale(GDS),Mini-Mental State Examination(MMSE)and Clinical Dementia Rating(CDR).The diagnostic efficacy of serum IL-13 and Hcy in AD was evaluated by the ROC curve.Results The levels of folate,IL-13 and MMSE in the observation group were lower than those in the control group(P<0.05).However,the levels of Hcy,GDS and CDR were higher than those in the control group(P<0.05),and the remaining differences between the two groups were not statistically significant(P>0.05).Hcy,GDS and CDR levels in the moderate and severe group were higher than those in the mild group(P<0.05),while IL-13,folate levels and MMSE scores were lower than those in the mild group(P<0.05).Correlation analysis showed that serum IL-13 was positively correlated with folate and MMSE scores(P<0.05).Serum Hcy was negatively correlated with folate and MMSE scores(P<0.05),and positively correlated with GDS and CDR scores(P<0.05).The area under the ROC curve of serum IL-13 and Hcy were larger,which had a good diagnostic performance for AD.Conclusions The levels of IL-13 and Hcy in AD patients may be related to cognitive impairment and dementia degree,and which can be considered as serological indexes for early diagnosis.
