持续正压通气对OSAHS合并牙周炎患者唾液TNF-α和IL-10含量影响

Effect of CPAP therapy on TNF-αand IL-10 levels of whole unstimulated saliva in patients with OSAHS associated chronic periodontitis

目的:探讨持续正压通气(continuous positive airway pressure,CPAP)治疗对阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea hypopnea syndrome,OSAHS)合并牙周炎患者唾液炎症介质的影响。方法:将本院2016年1月~2017年12月收治的63例阻塞性睡眠呼吸暂停低通气综合征伴牙周炎患者,随机分成观察组和对照组,其中观察组33例,给予牙周基础治疗联合CPAP治疗12周;对照组30例,仅予以牙周基础治疗,检测两组牙周临床指标。用ELISA法检测并比较观察组治疗前、后以及与对照组全唾液中TNF-α和IL-10水平差异。结果:观察组治疗后全唾液中TNF-α水平显著低于治疗前水平和对照组水平,差异有统计学意义(P<0.05),而IL-10水平较治疗前升高,差异有统计学意义(P<0.05)。结论:CPAP治疗能在一定程度上减轻OSAHS合并慢性牙周炎患者牙周炎症反应。

Objective:To investigate the effect of continuous positive airway pressure(CPAP)therapy on TNF-αand IL-10 levels in whole unstimulated saliva of patients with obstructive sleep apnea hypopnea syndrome(OSAHS)associated chronic periodontitis.Methods:Totally 63 patients with OSAHS associated chronic periodontitis selected from our hospital between January 2016 and December 2017 were divided into observation group(33 cases)and control group(30 cases)randomly,periodontal clinical index of two groups was examined.The two groups were given initial therapy,but observation group patients received the CPAP therapy for twelve weeks.TNF-αand IL-10 levels in whole unstimulated saliva were measured before and after therapy for twelve weeks.Results:TNF-αlevel of observation group was lower than before treatment and control group significantly(P<0.05),and IL-10 level in observation group was higher than before treatment significantly(P<0.05).Conclusion:CPAP therapy method can improve periodontal inflammation of patients with OSAHS associated chronic periodontitis to a certain extent.
