某院门急诊不合理处方的帕累托图分析

Pareto chart analysis of unreasonable prescription in outpatient and emergency department of a hospital

目的分析医院门急诊科室处方,了解不合理处方存在的问题,为临床合理用药提供参考。方法选取2019年乐山市人民医院门急诊科室处方点评资料,并利用帕累托图统计分析造成门急诊科室不合理用药的主要因素和次要因素。结果共抽取门急诊科室共28463张处方,其中不合理处方1957张,不合理率为6.88%。造成门急诊不合理用药的主要因素为用法用量不适宜、适应证不适宜、临床诊断书写不全,次要因素为遴选药品不适宜。结论临床药师应根据不合理处方类型制订针对性解决办法,提高医院处方质量,促进合理用药,保障患者用药安全。

Objective To analyze the prescriptions in outpatient and emergency departments of the hospital,understand the problems existing in unreasonable prescriptions,and provide reference for clinical rational drug use.Methods The data of prescription evaluation in outpatient and emergency departments of Leshan People′s Hospital in 2019 were selected.The main and secondary factors of irrational drug use in the Department of emergency and outpatient department were analyzed by Pareto chart.Results A total of 28463 prescriptions were drawn from outpatient and emergency departments,of which1957 were unreasonable,and the unreasonable rate was 6.88%.The main factors causing irrational drug use in outpatient and emergency department include inappropriate usage and dosage,inappropriate drug indications and incomplete clinical diagnosis writing;the secondary factor is inappropriate drug selection.Conclusion Clinical pharmacists should work out targeted solutions according to different types of irrational prescriptions,so as to improve the quality of hospital prescriptions,promote rational drug use,and ensure the safety of patients.
