SPECT/CT融合显像诊断早期缺血性股骨头坏死

摘要 目的:研究SPECT/CT融合显像诊断早期缺血性股骨头坏死(avascular necrosis of femoral head,ANFH)的诊断价值。方法:回顾性分析47例疑似为"缺血性股骨头坏死"成年患者而解剖影像学诊断无法确诊的患者行SPECT/CT融合显像检查,观察并分析其检查结果及图像特征。最终经过临床手术或者6个月随访确定检查诊断结果。计算并分析SPECT/CT融合显像对ANFH诊断的灵敏度、特异度和准确度。结果:58个股骨头SPECT/CT融合显像阳性49(其中48个为真阳性,1个为假阳性),阴性9个(真阴性6个,假阴性3个)。灵敏度、特异度、准确度分别为94.1%(48/51)、85.7%(6/7)、93.1%(54/58)。结论:SPECT/CT融合显像对早期缺血性股骨头坏死诊断有着较高的灵敏度和准确度,可为临床诊断提供宝贵的意见。 Objective:To evaluate the diagnostic value of SPECT/CT fusion imaging in early diagnosis of avascular necrosis of femoral head(ANFH).Methods:A retrospective analysis of 47 patients,a total of 58 emoral heads,who were doubted femoral head ischemic necrosis and who were not diagnosed by conventional imaging modalities,were examined with SPECT/CT fusion imaging.Finally,the results of the examination and diagnosis were confirmed by clinical surgery or 6 months follow-up.The sensitivity,specificity and accuracy of SPECT/CT fusion imaging for ANFH diagnosis were calculated and analyzed.Results:There were 49 positive SPECT/CT fusion imagines(48 true positive,1 false positive),negative 9(6 true negative,3 false negative).The sensitivity,specificity and accuracy of SPECT/CT fusion imaging were 94.1%(48/51),85.7%(6/7)and 93.1%(54/58),respectively.Conclusion:SPECT/CT fusion imaging has high sensitivity and accuracy for diagnosis of early avascular necrosis of femoral head,and can provide advice for clinical diagnosis.
