儿童良性阵发性眩晕32例分析

Analysis of benign paroxysmal vertigo in 32 children

目的分析儿童良性阵发性眩晕的临床特点,提高对该类疾病的认识,避免误诊漏诊。方法对32例良性阵发性眩晕患儿的发病年龄、眩晕特点、听力及前庭功能检查、偏头痛病史、运动病病史、相关家族史等进行回顾性分析。结果 32例患儿中男性19例,女性13例;年龄518岁,平均11.97岁。发病年龄217岁,平均9.09岁,男性比女性平均早发病2.69年。发作频率每年1次至每天数十次,持续时间最短<1 min,最长>72 h,伴畏光和(或)畏声8例,冷热试验异常4例,速度阶梯试验异常6例,c-VEMP异常2例。结论良性阵发性眩晕男性患儿偏多,眩晕发作比较孤立,常不伴耳鸣、耳闷、听力减退等耳部症状,多数为短暂眩晕发作,合并运动病病史、相关家族史比例较高,前庭功能异常并不能排除诊断。

OBJECTIVE To analyze the clinical characteristics of benign paroxysmal vertigo in children and to improve the understanding of such diseases and avoid misdiagnosis.METHODS Thirty-two children with benign paroxysmal vertigo were reviewed and the clinical characteristics of the children were evaluated.RESULTSThere were 19 boys and 13 girls with an average age of 11.97 years old.The average onset age was 9.09 years,meanwhile the average onset age of the boys was earlier than that of the girls.Frequency of attacks:1 time per year to dozens of times per day.Duration:<1 min to >72 h.Photophobia or phonophobia was found in 8 children.Caloric test was abnormal in 4 cases,Velocity step test was abnormal in 6 cases,cervical vestibular evoked myogenic potential was abnormal in 2 cases.CONCLUSION Benign paroxysmal vertigo is more common in boys,and the vertigo attacks is frequent and often not accompanied with tinnitus,aural fullness,hearing loss.Most of them are temporary vertigo attacks,with a higher proportion of motion sickness and related family history.Abnormal vestibular function cannot exclude this disease.
