针刺治疗中风后运动性失语系统评价和Meta分析

Systematic evaluation and Meta-analysis of acupuncture therapy for motor aphasia after stroke

目的:探讨针刺治疗中风后运动性失语的有效性和安全性。方法:计算机检索Pubmed、CNKI、CSPD、维普数据库(VIP)和Cochrane Library数据库,检索时间限定为2010年1月1日至2020年5月1日,收集以针刺疗法为主要方法治疗中风后运动性失语的相关随机对照试验(RCT),对纳入研究的相关结局指标进行Meta分析。结果:共纳入27项研究,其中有15项以ABC量表作为评价指标,8项以CRRCAE量表作为评价指标,Meta分析结果显示针刺疗法在ABC量表、CRRCAE量表等不同结局指标方面对中风后运动性失语有明确疗效,差异有统计学意义。结论:近10年随机对照试验表明针刺可显著缓解中风后运动性失语各方面指标,可继续运用于临床,但由于部分文献质量较低,未对针刺治疗后的有效性和安全性进行长期随访,导致不能评估针刺疗法对中风后运动性失语的长期疗效,故建议在今后应注重开展多中心、大样本的研究,对研究过程中的质量严格控制,统一治疗过程中可能影响结局数据的因素,进而开展高质量的临床研究。

Objective:To systematically evaluate the efficacy and safety of acupuncture in the treatment of motor aphasia after stroke.Methods:The computer retrieval Pubmed,CNKI,CSPD,weipu database(VIP)and the Cochrane Library database,retrieval time limit for the 2010 Jan 1 solstice May 1,2020,collected acupuncture as the main method in the treatment of motor aphasia after stroke related randomized controlled trial(RCT),research on into efficient and ABC,CRRCAE,CFCP,NIHSS outcome indexes such as Meta analysis.Results:27 study participants included,with a total of 15 with ABC inventory as evaluation index,eight CRRCAE scale as evaluation index,Meta analysis results showed that acupuncture in ABC,CRRCAE,different outcome indicators,such as the motor aphasia after stroke has clear curative effect,the difference was statistically significant.Conclusion:Nearly 10 years randomized controlled trials indicate that acupuncture can significantly relieve motor aphasia after stroke index,can continue to use in clinical,but due to the low part of the literature on quality,not on the effectiveness and safety of long-term follow-up after acupuncture treatment,lead to can not evaluate the long-term efficacy of acupuncture therapy on motor aphasia after stroke,therefore,suggest that in the future should pay attention to carry out multicenter,large sample study,to study the quality in the process of strict control,may affect the outcome data in the process of unified treatment factors,and then to carry out high quality clinical research.
