肠道益生菌联合纳洛酮对重症手足口病患儿免疫球蛋白水平及肠黏膜屏障功能的影响 被引量:1

摘要 目的探讨肠道益生菌联合纳洛酮治疗重症手足口病的疗效,以及对患儿免疫球蛋白水平及肠黏膜屏障功能的影响。方法选取医院2018年3月至2020年3月收治的重症手足口病患儿112例,按随机数字表法分为观察组和对照组,各56例。两组患儿均予常规治疗,观察组患儿加用肠道益生菌联合纳洛酮治疗,均连续治疗1周。结果观察组患儿的总有效率为89.29%,显著高于对照组的73.21%(P<0.05);观察组患儿的皮疹消退、退热、易惊消失、肢体震颤消失及住院的时间均显著短于对照组(P<0.05)。治疗后,观察组患儿的免疫球蛋白A、免疫球蛋白G、免疫球蛋白M水平均显著高于对照组(P<0.05),二胺氧化酶、D-乳酸、内毒素水平均显著低于对照组(P<0.05)。观察组和对照组的不良反应发生率相当(14.29%比8.93%,P>0.05)。结论肠道益生菌联合纳洛酮治疗重症手足口病疗效较好,可缩短患儿的临床症状消退时间,提高免疫球蛋白水平,改善肠道黏膜屏障功能,且安全性较好。 Objective To investigate the effect of intestinal probiotics combined with naloxone on immunoglobulin level and intestinal mucosal barrier function in children with severe hand-foot-and-mouth disease(HFMD).Methods A total of 112 patients with severe HFMD admitted to our hospital from March 2018 to March 2020 were randomly selected and divided into the observation group and the control group,56 cases in each group.The patients in the two groups were given routine treatment,on this basis,the patients in the observation group were given intestinal probiotics combined with naloxone.Both groups were continuously treated for one week.Results The total effective rate of the observation group was 89.29%,which was significantly higher than 73.21% of the control group(P<0.05).The time of rash subsided,fever subsided,tremor disappeared and hospitalization in the observation group was significantly shorter than that in the control group(P<0.05).After treatment,the levels of immunoglobulin A,immunoglobulin G and immunoglobulin M in the observation group were significantly higher than those in the control group(P<0.05),while the levels of diamine oxidase,D-lactic acid and endotoxin in the observation group were significantly lower than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was similar to that in the control group(14.29%vs.8.93%,P>0.05).Conclusion Intestinal probiotics combined with naloxone are effective and safe in the treatment of children with severe HFMD,which can shorten the disappearance time of clinical symptoms,improve the levels of immunoglobulin and the intestinal mucosal barrier function.
