呼吸道不同血清型别腺病毒感染患儿外周血T淋巴细胞亚群水平变化研究

Changes of T-lymphocyte subsets in peripheral blood of children with adenovirus infection of different serotypes in respiratory tract

目的:探讨呼吸道不同血清型别腺病毒感染患儿外周血T淋巴细胞亚群水平的变化情况。方法:选取67例呼吸道腺病毒感染患儿(研究组),另选取同期53例体检健康儿童(对照组),均行外周血T淋巴细胞亚群(CD_(3)^(+)、CD_(4)^(+)、CD_(8)^(+)、CD_(4)^(+)/CD_(8)^(+))水平检测。另对研究组毒株和(或)阳性标本行DNA提取,统计呼吸道腺病毒感染的不同血清型别,并分析各血清型别腺病毒感染患儿的外周血T淋巴细胞亚群水平变化情况。结果:研究组外周血CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)水平均较对照组降低,差异具有统计学意义(P<0.05);研究组外周血CD_(8)^(+)水平与对照组比较,差异无统计学意义(P>0.05);在67例呼吸道腺病毒感染患儿中,3型腺病毒35例(52.24%),7型腺病毒21例(31.34%),其他型腺病毒11例(16.42%);CD_(8)^(+)在研究组各血清型别间比较差异无统计学意义(均P>0.05);CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)在研究组各血清型别间比较有统计学差异(均P<0.05),3型、7型腺病毒感染患儿外周血CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)均低于其他型别腺病毒感染,7型腺病毒感染患儿CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)均低于3型腺病毒感染,差异均有统计学意义(均P<0.05)。结论:呼吸道腺病毒感染患儿外周血T淋巴细胞亚群中CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)水平均异常降低,其腺病毒感染型别以3型、7型为主,且3型、7型腺病毒感染患儿外周血CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)水平较其他型别下降明显,尤其是7型腺病毒感染降低最为严重。

Objective:To investigate the changes of T lymphocyte subsets in peripheral blood of children with adenovirus infection of different serotypes in respiratory tract.Methods:67 children with respiratory tract adenovirus infection were selected as study group,and the 53 healthy children during the same period were selected as control group.The levels of T lymphocyte subsets(CD_(3)^(+),CD_(4)^(+),CD_(8)^(+),CD_(4)^(+)/CD_(8)^(+)) in peripheral blood were detected.In addition,DNA were extracted from the virus strains and(or)positive samples of the study group,and different serotypes of respiratory tract adenovirus infection were counted,then the changes of T-lymphocyte subsets in peripheral blood of children with adenovirus infection of each serotypes were analyzed.Results:The peripheral blood CD_(3)^(+),CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+) levels of the study group were lower than those of the control group(all P<0.05).There was no significant difference in the level of CD_(8)^(+)in peripheral blood between the study group and the control group(P>0.05).Among 67 children with respiratory tract adenovirus infection,there were 35 cases with adenovirus type 3(52.24%),and there were 21 cases with adenovirus type 7(31.34%),and other adenovirus types were 11 cases(16.42%).There was no significant difference in CD_(8)^(+) level among serotypes in the study group(P>0.05).There were significant differences in CD_(3)^(+),CD_(4)^(+) and CD_(4)^(+)/CD_(8)^(+) levels among serotypes in the study group(all P<0.05).The levels of CD_(3)^(+),CD_(4)^(+) and CD_(4)^(+)/CD_(8)^(+) in peripheral blood of children infected with type 3 and 7 adenovirus were lower than those of other types adenovirus,of which the children infected with type 7 adenovirus were lower than those of the children infected with type 3 adenovirus(all P<0.05).Conclusion:The levels of CD_(3)^(+),CD_(4)^(+) and CD_(4)^(+)/CD_(8)^(+) in peripheral blood T-lymphocyte subsets of children with respiratory tract adenovirus infection are abnormally reduced,and the main types of adenovirus infection are type 3 and type 7,and the levels of CD_(3)^(+),CD_(4)^(+) and CD_(4)^(+)/CD_(8)^(+) in peripheral blood of children with type 3 and type 7 adenovirus infection are significantly lower than those of other types,especially type 7 adenovirus infection.
