间歇经口至食管管饲法联合多奈哌齐片治疗阿尔茨海默病致吞咽障碍患者应用评价

Intermittent oro-esophageal tube feeding combined with donepezil in the treatment of dysphagia caused byAlzheimer’s diseases

目的观察间歇经口至食管管饲法(IOE)联合多奈哌齐对阿尔茨海默病(AD)吞咽障碍患者认知功能、营养状况及日常生活能力的影响。方法符合AD致吞咽障碍的患者随机分为2组,所有患者基础治疗相同,对照组采用IOE改善营养状况,观察组加用多奈哌齐,比较2组患者干预前后MMSE评分、MoCA评分、体重指数(BMI)、血红蛋白(Hb)及日常生活活动能力(ADL)评分。结果干预前2组患者MMSE评分、MoCA评分、Barthel指数评分、BMI、Hb比较差异均无统计学意义(P>0.05)。干预后2组患者MMSE评分、MoCA评分、Barthel指数评分、BMI、Hb均较干预前增加,差异均有统计学意义(P<0.05);与对照组比较,干预后观察组MMSE评分、MoCA评分、Barthel指数评分、BMI、Hb改善更显著,差异均有统计学意义(P<0.05)。结论IOE联合多奈哌齐可有效改善AD患者的认知功能、营养状况及日常生活能力。

Objective To observe the effect of intemittent oro-esophageal tube feeding(IOE)combined with donepezil on the cognitive function,nutritional status and daily living ability(ADL)of dysphagia caused by Alzheimer’s diseases(AD).Methods Patients with dysphagia due to AD were divided into two groups.The basic treatment of the two groups was the same.The control group used IOE to improve nutrition.The observation group added donepezil tablets.The MMSE score,MoCA score,body mass index(BMI),hemoglobin(Hb)and ADL scores of the two groups of patients before and after treatment were compared.Results There were no statistically significant differences in MMSE score,MoCA score,Barthel index score,BMI,and HB(P>0.05).After intervention,MMSE score,MoCA score,Barthel index score,BMI,and HB of two groups increased significantly before intervention,and the differences were statistically significant(P<0.05).Compared with the control group,the improvement of MMSE score,MoCA score,Barthel index score,BMI,HB of the observation group were more significant,and the differences were statistically significant(P<0.05).Conclusion Intermittent oro-esophageal tube feeding combined with donepezil can effectively improve the cognitive function,nutritional status and daily living ability of Alzheimer’s disease patients.
