Application of Ultrasound-Guided Thoracic Paravertebral Block in Thoracic Surgery for Patients with Cardiovascular and Cerebrovascular Diseases

摘要 目的探讨超声引导胸椎旁神经阻滞(TPVB)在心脑血管病患者胸科手术中的应用效果。方法60例心脑血管病患者随机分为两组各30例。两组均给予胸科手术治疗,对照组实施全身麻醉,观察组实施超声引导TPVB联合全身麻醉。比较两组的苏醒质量及生命体征。结果两组的苏醒时间、镇静评分比较,差异无统计学意义(P>0.05);观察组术后2 h的疼痛程度评分显著低于对照组,拔管时间显著短于对照组(P<0.05)。两组T0时的MAP、HR、SpO_(2)、RR比较,差异无统计学意义(P>0.05);观察组T1、T2、T3时的MAP、HR、SpO_(2)、RR均显著优于对照组(P<0.05)。结论超声引导TPVB应用于心脑血管病患者胸科手术的临床效果显著,可有效改善患者的苏醒质量,稳定生命体征。 Objective To explore the application effect of ultrasound-guided thoracic paravertebral block(TPVB)in thoracic surgery for patients with cardiovascular and cerebrovascular diseases.Methods 60 patients with cardiovascular and cerebrovascular diseases were randomly divided into two groups,with 30 cases in each group.Both groups were treated with thoracic surgery.The control group received general anesthesia,and the observation group received ultrasound-guided TPVB combined with general anesthesia.The waking quality and vital signs were compared between two groups.Results No significant difference was found in the recovery time and sedation score between two groups(P>0.05);The pain degree score at 2 h after surgery of observation group was significantly lower than that of control group,and the extubation time was significantly shorter than that of control group(P<0.05).At T0,no significant difference was found in MAP,HR,SpO_(2) and RR between two groups(P>0.05);At T1,T2 and T3,MAP,HR,SpO_(2) and RR of observation group were significantly better than those of control group(P<0.05).Conclusions Ultrasound-guided TPVB applied in thoracic surgery for patients with cardiovascular and cerebrovascular diseases has significant clinical effect,and can effectively improve patients'waking quality and stabilize vital signs.
