沙库巴曲缬沙坦与缬沙坦氨氯地平对心衰伴肾功能不全患者心功能及肾小球滤过率的影响比较

Comparison on the Impact of Sacubitril/Valsartan vs.Valsartan/Amlodipine on the Cardiac Function and Glomerular Filtration Rate of Patients with Heart Failure Complicated with Renal Insufficiency

目的比较沙库巴曲缬沙坦与缬沙坦氨氯地平对心衰伴肾功能不全患者心功能及肾小球滤过率的影响。方法85例心衰伴肾功能不全患者随机分为两组。在常规治疗基础上,对照组口服缬沙坦氨氯地平片,观察组口服沙库巴曲缬沙坦片。比较两组治疗前后的心功能及肾小球滤过率(eGFR)。结果两组治疗后的LVEF水平均上升,LVESD水平均下降,且观察组的LVEF水平显著高于对照组,LVESD水平显著低于对照组(P<0.05)。两组治疗前后的eGFR水平比较,差异无统计学意义(P>0.05)。结论相对于缬沙坦氨氯地平,沙库巴曲缬沙坦治疗心衰伴肾功能不全患者具有更好的心功能改善效果,对肾功能的影响小。

Objective To compare the impact of sacubitril/valsartan vs.valsartan/amlodipine on the cardiac function and glomerular filtration rate of patients with heart failure complicated with renal insufficiency.Methods 85 patients with heart failure complicated with renal insufficiency were randomly divided into two groups.On the basis of conventional treatment,the control group received valsartan/amlodipine tablets orally,and the observation group received sacubitril/valsartan tablets orally.The cardiac function and glomerular filtration rates(eGFR)before and after treatment were compared between two groups.Results After treatment,both groups had increased LVEF level and decreased LVESD level;The LVEF level of observation group was significantly higher than that of control group,and the LVESD level was significantly lower than that of control group(P<0.05).The eGFR levels before and after treatment showed no statistical difference between two groups(P>0.05).Conclusions Compare with valsartan/amlodipine,sacubitril/valsartan in the treatment of patients with heart failure complicated with renal insufficiency has better improvement effect in cardiac function,with small impact on renal function.
