摘要
目的分析血液透析患者自体动静脉内瘘血栓形成的影响因素。方法回顾性分析2017年2月至2019年10月期间在我院进行血液透析的196例患者的临床资料,采用单因素、多因素分析探讨血液透析患者自体动静脉内瘘血栓形成的影响因素。结果196例血液透析患者中自体动静脉内瘘血栓形成18例,占比9.18%。单因素、多因素Logistic回归分析显示,超滤过多过快、血液高凝状态、血红蛋白>130 g/L、糖尿病史、高甘油三酯血症史、内瘘建立或使用不当均为血液透析患者自体动静脉内瘘血栓形成的危险因素(OR>1,P<0.05)。结论超滤过多过快、血液高凝状态、血红蛋白>130 g/L、糖尿病史、高甘油三酯血症史、内瘘建立或使用不当均是血液透析患者动静脉内瘘血栓形成的危险因素,临床应对相关因素进行干预以减少自体动静脉内瘘血栓形成的发生。
Objective To analyze the influencing factors of thrombosis of autogenous arteriovenous fistula in hemodialysis patients.Methods The clinical data of 196 patients with hemodialysis in our hospital from February 2017 to October 2019 were retrospectively analyzed.Univariate analysis and multivariate analysis were used to explore the influencing factors of thrombosis of autogenous arteriovenous fistula in hemodialysis patients.Results Among 196 hemodialysis patients,thrombosis of autogenous arteriovenous fistula occurred in 18 patients,accounted for 9.18%.Univariate analysis and multivariate Logistic regression analysis showed that overmuch and overquick ultrafiltration,blood hypercoagulability,hemoglobin>130 g/L,history of diabetes mellitus,history of hypertriglyceridemia,inappropriate establishment or use of internal fistula were all risk factors for thrombosis of autologous arteriovenous fistula in hemodialysis patients(OR>1,P<0.05).Conclusions Overmuch and overquick ultrafiltration,blood hypercoagulability,hemoglobin>130 g/L,history of diabetes mellitus,history of hypertriglyceridemia,inappropriate establishment or use of internal fistula are all risk factors for thrombosis of autologous arteriovenous fistula in hemodialysis patients.Relevant factors should be intervened to reduce the occurrence of thrombosis of autologous arteriovenous fistula in clinic.
