联合手术治疗高龄闭角型青光眼合并白内障疗效观察

Clinical observation of combined surgery in the treatment of elderly patients with angle closure glaucoma and cataract

目的观察联合手术方式治疗高龄闭角型青光眼合并白内障患者的临床效果。方法研究对象选取自2017年6月至2019年6月安徽医科大学附属宿州医院收治的57例(57只眼)高龄闭角型青光眼合并白内障患者,并分为对照组和观察组。其中,对照组26例,行单纯小梁切除术;观察组31人,接受联合手术治疗(包括小梁切除术、超声乳化白内障吸除术及人工晶状体植入术)。比较两组患者的疗效及并发症发生情况。结果两组患者术后的视力、眼压与前房深度均较术前有显著改善,但观察组的治疗效果优于对照组,差异均有统计学意义(P<0.05)。同时,观察组患者术后并发症的发生率为也显著低于对照组,差异有统计学意义(P<0.05)。结论通过联合手术方式治疗高龄闭角型青光眼合并白内障具有良好的临床效果,同时可减少并发症的发生。

Objective To investigate the clinical effect of combined surgery in the treatment of elderly patients with angle-closure glaucoma and cataract.Methods A total of 57 elderly patients with angle-closure glaucoma complicated with cataract admitted to the Suzhou Municipal Hospital from June 2017 to June 2019 were selected.All patients were divided into two groups:group A(n=26)treated with simple trabeculectomy and group B(n=31)received combined surgery(including trabeculectomy,phacoemulsification cataract extraction and intraocular lens implantation).The efficacy and complications were compared between the two groups.Results The visual acuity,intraocular pressure and anterior chamber depth of the two groups of patients were significantly improved compared with that before the operation.However,the treatment effect of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).At the same time,the incidence of postoperative complications in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusions The combined surgery can effectively improve vision and reduce the incidence of postoperative complications for elderly patients with angle-closure glaucoma and cataract.
