加味定经汤治疗同种免疫复发性流产的机制研究

目的探讨加味定经汤治疗同种免疫复发性流产的机制。方法选取2019年2—9月我院就诊的同种免疫复发性流产患者106例为研究对象,根据治疗方法不同分为观察组和对照组,各53例,观察组采用加味定经汤进行治疗,对照组行淋巴细胞主动免疫治疗。观察两组治疗14周前后自然杀伤细胞(NK)、辅助型T细胞1/辅助型T细胞2(Th1/Th2),比较治疗前后观察组中医证候总评分,对比两组在妊娠后NK、Th1/Th2,追踪随访其妊娠结局并比较妊娠活产率。结果治疗前两组NK、Th1/Th2比较,差异无统计学意义(P>0.05),治疗后观察组NK、Th1/Th2比对照组低,差异有统计学意义(P<0.05)。治疗后观察组中医证候总评分比治疗前低,差异有统计学意义(P<0.05)。妊娠后,观察组NK、Th1/Th2比对照组低,妊娠活产率比对照组高,差异有统计学意义(P<0.05)。结论加味定经汤治疗同种免疫复发性流产效果较显著,有效改善患者临床症状并对妊娠结局有积极影响,为同种免疫复发性流产患者的治疗提供参考。

Objective To investigate the mechanism of modified Dingjing Decoction in the treatment of homoimmune recurrent abortion.Methods A total of 106 patients diagnosed as homo-immune recurrent abortion in our hospital from February to September 2019 were selected as the research objects.They were divided into the observation group(n=53)and the control group(n=53)according to the different treatment methods.The observation group was treated with modified Dingjing Decoction,while the control group was treated with lymphocyte active immunotherapy.The natural killer cells(NK),helper T cell 1/helper T cell 2(Th1/Th2)of the two groups before and after 14 weeks of treatment were observed.The total scores of traditional Chinese medicine(TCM)syndromes of the observation group before and after treatment were compared.The NK,Th1/Th2 of the two groups after pregnancy were compared.Meanwhile,the pregnancy outcome was followed up and the live birth rate of pregnancy was compared.Results Before treatment,there were no statistically significant differences in NK,Th1/Th2 between the two groups(P>0.05).After treatment,NK and Th1/Th2 in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).After treatment,the total score of TCM syndromes in the observation group was lower than that before treatment,with statistically significant difference(P<0.05).After pregnancy,NK and Th1/Th2 in the observation group were lower than those in the control group,and the live birth rate of pregnancy was higher than that in the control group,with statistically significant differences(P<0.05).Conclusion Modified Dingjing Decoction has a remarkable efficacy on the treatment of homo-immune recurrent abortion,which effectively improves the clinical symptoms of patients and has a positive impact on pregnancy outcome,as well as provides a reference for the treatment of patients with homo-immune recurr ent abortion.
