人文关怀护理结合情绪释放法对乳腺癌术后化疗患者抑郁、焦虑及生活质量的影响

Analysis on the impacts of humanistic care combined with emotional release method on depression,anxiety and quality of life of patients with breast cancer undergoing postoperative chemotherapy

目的探讨人文关怀护理结合情绪释放法对乳腺癌术后化疗患者抑郁、焦虑及生活质量影响效果,并探讨其对患者治疗依从性及护理服务质量满意度的影响效果。方法选择2018年4月至2020年6月在深圳市龙华区人民医院和宝安区福永人民医院确诊术后化疗的乳腺癌患者102例,随机分为对照组和研究组,每组各51例。对照组采用常规护理,研究组在对照组护理基础上采用人文关怀结合情绪释放法,连续干预4周,分别于干预前后采用焦虑自评量表(SAS)、抑郁自评量表(SDS)及生活质量自评量表(QoLQ)对患者进行SAS、SDS及QoLQ评分,并对患者的治疗依从率及护理服务质量满意度进行问卷调查。结果干预前两组患者的SAS、SDS评分之间差异均无统计学意义(P>0.05),干预后研究组的SAS、SDS评分明显低于干预前及对照组干预后,差异均有统计学意义(P<0.05),而对照组的SAS、SDS评分虽比干预前略有降低,但差异均无统计学意义(P>0.05)。干预前两组患者的生理、心理、躯体及社会等生活质量功能评分之间差异均无统计学意义(P>0.05),干预后研究组的生理、心理、躯体及社会等功能评分均明显高于干预前和对照组干预后,差异均有统计学意义(P<0.05),而对照组的生理、心理、躯体及社会等生活质量功能评分比干预前略有升高,但差异均无统计学意义(P>0.05)。研究组患者治疗总依从率为96.08%,明显高于对照组的78.43%,差异有统计学意义(P<0.05)。研究组患者对护理服务质量总的满意度为92.16%,明显高于对照组的74.51%,差异有统计学意义(P<0.05)。结论人文关怀护理结合情绪释放法能明显改善乳腺癌术后化疗患者抑郁和焦虑等负面情绪。同时还能提高患者的生活质量、治疗依从率及护理服务质量满意度,值得临床推广应用。

Objective To investigate the impacts of humanistic care combined with emotional release method on depression,anxiety and quality of life(QoL)of patients with breast cancer undergoing postoperative chemotherapy,and to investigate its effects on treatment compliance and nursing service quality satisfaction.Methods A total of 102 patients with breast cancer undergoing chemotherapy after gynecological diagnosis in Shenzhen Longhua People's Hospital and Fuyong People's Hospital from April 2018 to June 2020 were randomly divided into the control group(n=51)and the study group(n=51).The control group was treated with conventional nursing,while the study group was treated with humanistic care combined with emotional release method on the basis of the control group.The patients were continuously intervened for 4 weeks.The patients were scored by self-rating anxiety scale(SAS),selfrating depression scale(SDS)and qualities of life with the quality(QoLQ)before and after intervention,respectively.And the treatment compliance rate and nursing service quality satisfaction of patients were investigated by questionnaire.Results There were no statistically significant differences in SAS and SDS scores between the two groups before intervention(P>0.05),while the SAS and SDS scores of the study group after intervention were significantly lower than those before intervention and those of the control group after intervention,all with statistically significant differences(P<0.05).Meanwhile,the SAS and SDS scores of the control group were slightly lower than those before intervention,but all without statistically significant differences(P>0.05).Before intervention,there were no statistically significant differences in the scores of QoL,such as physiological,psychological,physical and social functions between the two groups of patients(P>0.05).After intervention,the scores of QoL,such as physiological,psychological,physical and social functions of the study group were all significantly higher than those before intervention and those of the control group after intervention,with statistically significant differences(P<0.05).Meanwhile,the scores of QoL,such as physiological,psychological,physical and social functions of the control group were slightly higher than those before intervention,but all without statistically significant differences(P>0.05).The total compliance rate of patients in the study group was 96.08%,which was significantly higher than 78.43%in the control group,with statistically significant difference(P<0.05).The total satisfaction with the nursing service quality of patients in the study group was 92.16%,which was significantly higher than 74.51%in the control group,with statistically significant difference(P<0.05).Conclusion Humanistic care combined with emotional release method can obviously improve the negative emotions such as depression and anxiety of patients with breast cancer undergoing postoperative chemotherapy.At the same time,it can also improve QoL of patients,treatment compliance rate and nursing service quality satisfaction,which is worthy of cl inical promotion and application.
