天津市河东区高血压患者服药依从性现状及其影响因素

Status quo of medication compliance and analysis of influencing factors in hypertensive patients in Hedong District of Tianjin

【目的】探讨天津市河东区18岁以上高血压患者药物治疗依从性现状及其影响因素,为后续的政策制定和干预研究提供依据。【方法】2018年9月采用整群随机抽样的方法对天津市河东区18岁以上高血压患者进行社区诊断调查,对参与社区诊断调查人群中采用药物治疗的3 912例高血压患者,进行服药依从性调查,对服药依从性的相关因素进行多因素Logistics回归分析。【结果】本研究调查的高血压患者依从性好的比例为43.63%(1 707例)。不同年龄、BMI、家庭月收入、购买社会保险、知信行高血压必要长期服药及知信行高血压遵从医嘱对服药依从性差异有统计学意义(P<0.05)。多因素Logistics回归分析结果显示:年龄、购买社会保险、知信行高血压有必要长期服药及知信行高血压遵从医嘱的情况是社区高血压患者服药依从性的影响因素(P<0.05)。【结论】天津市河东区18岁以上高血压患者服药依从性较低。应加强对社区高血压患者服药依从性的重视,针对其影响因素和特点采取多种措施进行预防与干预,以提高社区高血压患者医治效果。

【Objective】To investigate the status quo of medication and its influencing factors in hypertensive patients over 18 years in Hedong District of Tianjin, and provide the evidences for the subsequent policy-making and intervention research.【Methods】A questionnaire survey was conducted among 3 912 hypertensive patients recruited with cluster random sampling in Hdong District of Tianjin. Multivariate Logistic regression analysis was used to analyze the factors that may affect the medication compliance.【Results】There were 43.63%(1 707/3 912) of hypertensive patients who complied with their medical regimens well. In the univariate analysis,patients’ medication compliance was associated with the factors of age(P=0.001), BMI, monthly household income, purchase of social insurance, long-term medication necessary for hypertension and compliance with medical advice(P<0.05). In the multivariate analysis,the factors of age, purchase of social insurance, long-term medication necessary for hypertension and compliance with medical advice were identified to independently affect medicationcompliance in hypertensive patients(P<0.05). 【Conclusion】The medication compliance of hypertension in patients over 18 years old in Hdong District of Tianjin is low. We should pay more attention to the medication compliance of hypertension patients in community, and take various measures to prevent and intervene hypertension according to the influencing factors and characteristics, so as to improve the therapeutic effect of hypertension in community patients.
