摘要
目的分析CT下测量定位在多发性肋骨骨折切开复位内固定手术切口选择时的应用价值。方法 80例多发性肋骨骨折切开复位内固定手术患者,根据数字随机法分为对照组和观察组,每组40例。对照组给予多发性肋骨骨折切开复位内固定手术常规切口治疗,观察组给予CT下测量定位切口切开复位内固定治疗。比较两组手术时间、手术切口长度、切口准确率、优良率以及治疗前后视觉模拟评分法(VAS)评分、生活质量评分。结果观察组手术时间(65.21±1.34)min、手术切口长度(4.21±1.51)cm均短于对照组的(84.51±2.95)min、(8.25±2.18)cm,差异均具有统计学意义(P<0.05)。治疗后,两组患者VAS评分均较治疗前降低,生活质量评分均较治疗前升高,且观察组患者VAS评分(1.01±0.13)分低于对照组的(2.42±0.42)分,生活质量评分(94.56±3.01)分高于对照组的(83.21±2.55)分,差异均具有统计学意义(P<0.05)。观察组切口准确率97.50%、优良率97.50%均高于对照组的77.50%、75.00%,差异均具有统计学意义(P<0.05)。结论 CT下测量定位在多发性肋骨骨折切开复位内固定手术切口选择时的应用价值高,可提高切口准确度,并缩短切口长度及手术时间,减轻患者疼痛,促进患者生活质量提高。
Objective To analyze the practical value of CT measurement and positioning in the selection of surgical incisions for open reduction and internal fixation of multiple rib fractures.Methods A total of 80 patients with multiple rib fractures undergoing open reduction and internal fixation were divided into control group and observation group according to random numerical table,with 40 cases in each group.The control group was treated with conventional incision treatment of open reduction and internal fixation,and the observation group was treated with CT measurement and positioning of open reduction and internal fixation.The operation time,incision length,incision accuracy,excellent rate,visual analogue scale(VAS)score before and after treatment,and quality of life score were compared between the two groups.Results The operation time(65.21±1.34)min and incision length(4.21±1.51)cm of the observation group were shorter than(84.51±2.95)min and(8.25±2.18)cm of the control group,and the difference was statistically significant(P<0.05).After treatment,the VAS score of the two groups was lower than those before treatment,and quality of life score was higher than those before treatment;the VAS score(1.01±0.13)points of the observation group was lower than(2.42±0.42)points of the control group,and quality of life score(94.56±3.01)points was higher than(83.21±2.55)points of the control group.All the difference was statistically significant(P<0.05).The incision accuracy 97.50%and excellent rate 97.50%of the observation group were higher than 77.50%and 75.00%of the control group,and the difference was statistically significant(P<0.05).Conclusion CT measurement and positioning is of high value in the selection of incision for open reduction and internal fixation of multiple rib fractures.It can improve the accuracy of incision,shorten the length of incision and operation time,relieve pain and improve the quality of life of patients.
