子宫内膜癌病理分化程度与ADC值相关性研究

Study on the correlation between the degree of pathological differentiation of endometrial carcinoma and DWI ADC value

目的:分析子宫内膜癌病理分化程度与磁共振弥散加权成像(DWI)的表观扩散系数(ADC)值相关性,为临床制定子宫内膜癌的治疗方案提供指导。方法:选定本院2018年8月—2020年8月收治的70例子宫内膜癌患者,根据分化程度,分为低分化组(32例)、中分化组(24例)、高分化组(14例),均给予DWI成像,测量ADC值,对比不同分化程度组ADC值,Pearson分析病理分化程度与ADC值的相关性。结果:常规平扫:T_(1)WI序列下肿瘤病灶出现等信号或稍低信号,T_(2)WI序列下出现等信号或稍高信号。T_(1)WI序列下液化坏死区域出现低信号,T_(2)WI序列下出现高信号。肿瘤病灶DWI序列下出现低信号或明显高信号。低分化组ADC值比中分化组低,中分化组ADC值比高分化组低,P<0.05(差异均具有统计学意义)。子宫浆液性乳突状腺癌组、透明细胞癌组、腺癌组ADC值比较,P>0.05。病理分化程度与DWI的ADC值呈正相关性,P<0.05(r=0.628)。结论:子宫内膜癌患者病理分化程度与ADC值呈正相关性,及时检测DWI的ADC值,对治疗方案的制定具有重要的指导意义。

Objective: To analyze the correlation between the degree of pathological differentiation of endometrial cancer and apparent diffusion coefficient(ADC), which is derived from diffusion weighted imaging(DWI), and to provide guidance for clinical treatment of endometrial carcinoma. Methods: Seventy endometrial cancer patients in our hospital from August 2018 to August 2020 were enrolled. According to the degree of differentiation, they were divided into poorly differentiated group(32 cases), moderately differentiated group(24 cases), and highly differentiated group(14 cases). All of the patients underwent DWI scan. ADC values were measured, and ADC values of different differentiation levels were compared. The correlation between pathological differentiation and ADC value was analyzed. Results: The lesions were isointense or slightly hypointense on T_(1)WI,isointense or slightly hyperintense on T_(2)WI. The necrosis area was low signal on the T_(1)WI and high signal on T_(2)WI. The lesions were low signal or high signal on DWI. The ADC value of the low-differentiation group was lower than that of the middle-differentiation group, and the ADC value of the middle-differentiation group was lower than that of the high-differentiation group, P <0.05(all differences were statistically significant). The ADC values of uterine serous mastoid adenocarcinoma group, clear cell carcinoma group and adenocarcinoma group were compared, and P>0.05. The degree of pathological differentiation was positively correlated with ADC value, P<0.05(r =0.628). Conclusion: The degree of pathological differentiation of endometrial cancer is positively correlated with ADC value. Timely detection of ADC value has important guiding significance for the formulation of treatment plans.
