精密附着体修复后基牙牙周状况的临床研究

摘要 目的探讨精密附着体修复后对远中基牙牙周组织的影响,为其临床应用提供理论基础。方法25例KennedyⅠ型和Ⅱ型牙列缺损患者,均使用精密附着体修复,将缺牙区相邻的两个前磨牙选作基牙,在修复前,修复后1、3、6个月,分别采集基牙的龈沟液(GCF)并称重,比较修复前后基牙牙槽骨高度的变化。结果修复后1个月的GCF量(0.34±0.06)mg与修复前的(0.32±0.07)mg比较,差异无统计学意义(P>0.05);修复后3个月的GCF量为(0.49±0.13)mg、修复后6个月的GCF量为(0.50±0.13)mg,多于修复前及修复后1个月,差异具有统计学意义(P<0.05);修复后3个月与修复后6个月的GCF量比较,差异无统计学意义(P>0.05)。修复前后的牙槽骨高度比较,差异无统计学意义(P>0.05)。结论使用精密附着体修复牙列游离缺损时应注意维护基牙的牙周健康,强化口腔卫生宣传教育。 Objective To discuss the effect of precision attachment restoration on abutment periodontal condition,so as to provide theoretical basis for its clinical application.Methods All 25 patients with Kennedy typeⅠand typeⅡdentition defects all received precision attachment restoration,and the two adjacent premolars in the edentulous area were selected as abutments.Before restoration,1,3,and 6 months after restoration,the gingival crevicular fluid(GCF)of the abutment was collected and weighed to compare the changes in alveolar bone height of abutments before and after restoration.Results 1 month after restoration,the GCF volume(0.34±0.06)mg had no statistically significant difference compared with(0.32±0.07)mg before restoration(P>0.05).The GCF volume at 3 and 6 months after restoration were(0.49±0.13)and(0.50±0.13)mg,which were more than those before restoration and 1 month after restoration,and the difference was statistically significant(P<0.05).There was no statistically significant difference in GCF volume between 3 months after restoration and 6 months after restoration(P>0.05).There was no statistically significant difference in alveolar bone height before and after restoration(P>0.05).Conclusion It is necessary to examine periodically after restored in order to keep the periodontal health of the abutment.Conclusion When using precision attachment to repair free dentition defect,more attention should be payed to maintain the periodontal health of abutment teeth and strengthen oral health education.
