摘要
目的探讨早期单核细胞数增高与肺炎支原体(Mycoplasma pneumoniae,MP)感染的相关性,为MP肺炎早期诊治提供临床依据.方法选取符合肺炎诊断标准住院患者513例,根据外周血单核细胞数分为外周血单核细胞增高组和外周血单核细胞正常组,采用金标法检测MP抗体(Ig G、Ig M),统计各组阳性病例数.在外周血单核细胞增高组中统计组内阳性病例数,并统计支原体感染阳性病例数在不同季节的分布情况.结果早期外周血单核细胞增高组支原体抗体检测阳性率明显高于外周血单核细胞正常组(χ^(2)=31.060,P<0.05),以冬春季患病率最高(χ^(2)=9.973,P<0.05),且在早期外周血单核细胞增高组中支原体抗体阳性率高达57.8%,而单核细胞正常组支原体感染阳性率仅为29.5%.结论儿童肺炎患者如早期单核细胞增高应高度警惕MP感染的可能,尤其在冬春季,需及时检测MP抗体,以指导早期临床诊断和治疗.
Objective To explore relationship between the early increase of monocyte count and mycoplasma pneumoniae(MP)infection,and provide clinical basis for early diagnosis and treatment of mycoplasma pneumoniae pneumonia.Method 513 inpatients were divided into two groups according to the number of monocytes in peripheral blood.Mycoplasma pneumoniae antibody was detected by gold standard method(IgG and IgM),and were used to count the number of positive cases in each group.In the peripheral blood mononuclear cell increase group,the number of positive cases in the group and the number of positive cases of mycoplasma infection were counted according to the season.Results The positive rate of mycoplasma antibody in the early peripheral blood mono-nucleosis group was significantly higher than that in the normal peripheral blood monocyte group(χ^(2)=31.060,P<0.05),and the highest prevalence rate was in winter and spring(χ^(2)=9.973,P<0.05).And in the early peripheral blood mononucleosis group,the positive rate of mycoplasma antibody was as high as 57.8%,while the positive rate of mycoplasma infection in the normal monocyte group was only 29.5%.Conclusion Children with early monocytosis of pneumonia should be highly alert to the possibility of mycoplasma pneumoniae infection,especially in winter and spring,it is necessary to detect mycoplasma pneumoniae antibody in time to guide early clinical diagnosis and treatment.
