摘要
目的探讨乙酰半胱氨酸联合有创-无创序贯通气治疗慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭患者的效果。方法选取2018年1月至2021年1月本院收治的65例COPD合并Ⅱ型呼吸衰竭患者为研究对象,按照随机数字表法将其分为观察组(33例)和对照组(32例)。对照组给予有创-无创序贯通气治疗,观察组在对照组基础上增加乙酰半胱氨酸治疗。比较两组患者的临床疗效、肺通气指标、血气指标及不良反应发生情况。结果观察组患者的治疗总有效率为93.94%,高于对照组的71.88%(P<0.05)。治疗后,观察组FVC、FEV_(1)%pred、PEF均大于对照组(P<0.05)。治疗后,观察组患者PaO_(2)、OI高于对照组,PaCO_(2)低于对照组(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论乙酰半胱氨酸联合有创-无创序贯通气治疗COPD合并Ⅱ型呼吸衰竭患者的效果显著,能够改善患者的肺通气指标和血气指标,且不会增加不良反应,可推荐于临床工作中。
Objective To explore the effect of acetylcysteine combined with invasive-noninvasive sequential ventilation on patients with chronic obstructive pulmonary disease(COPD)complicated with typeⅡrespiratory failure.Methods A total of 65 patients with COPD complicated with typeⅡrespiratory failure admitted in our hospital from January 2018 to January 2021 were selected as the research objects,and the patients were divided into observation group(33 cases)and control group(32 cases)according to random number table method.The control group was given invasive-noninvasive sequential ventilation treatment,and the observation group was given acetylcysteine treatment on the basis of the control group.The clinical efficacy,lung ventilation indexes,blood gas indexes and adverse reactions of the two groups were compared.Results The total effective rate of treatment of the observation group was 93.94%,which was higher than 71.88%of the control group(P<0.05).After treatment,FVC,FEV_(1)%pred and PEF in the observation group were greater than those in the control group(P<0.05).After treatment,PaO_(2) and OI in the observation group were higher than those in the control group,while PaCO_(2) in the observation group was lower than that in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Acetylcysteine combined with invasive-noninvasive sequential ventilation in the treatment of COPD patients complicated with typeⅡrespiratory failure has a significant effect,it can improve the lung ventilation indexes and blood gas indexes,and will not increase adverse reactions,which can be recommended in clinical work.
