无创正压通气治疗急性呼吸窘迫综合征的临床效果及对患者呼吸功能的影响

Clinical effect of non-invasive positive pressure ventilation in the treatment of acute respiratory distress syndrome and its influence on respiratory function of patients

目的探讨无创正压通气(NIPPV)治疗急性呼吸窘迫综合征(ARDS)的临床效果及对患者呼吸功能的影响。方法选取我院2017年1月至2020年1月收治的52例ARDS患者作为研究对象,按照治疗方式将其分为对照组与观察组,各26例。对照组实施有创机械通气治疗,观察组实施NIPPV治疗。比较两组的临床效果。结果观察组的ICU住院时间、总住院时间、机械通气持续时间均短于对照组(P<0.05)。治疗48 h后,两组的PaO_(2)、SaO_(2)、潮气量和分钟通气量均升高,PaCO_(2)及呼吸频率均降低,且观察组优于对照组(P<0.05)。观察组的并发症总发生率低于对照组(P<0.05)。治疗后,两组的舒适度评分均升高,APACHEⅡ评分均降低,且观察组优于对照组(P<0.05)。结论NIPPV能有效改善ARDS患者的呼吸功能,缩短住院时间,降低并发症发生率,值得临床推广应用。

Objective To explore the clinical effect of non-invasive positive pressure ventilation(NIPPV)in the treatment of acute respiratory distress syndrome(ARDS)and its influence on respiratory function of patients.Methods A total of 52 patients with ARDS admitted in our hospital from January 2017 to January 2020 were selected as the research objects,and the patients were divided into control group and observation group according to the treatment methods,with 26 cases in each group.The control group was treated with invasive mechanical ventilation,and the observation group was treated with NIPPV.The clinical effects of the two groups were compared.Results The ICU hospital stay,total hospital stay and duration of mechanical ventilation in the observation group were shorter than those in the control group(P<0.05).After 48 h of treatment,PaO_(2),SaO_(2),tidal volume and minute ventilation volume in the two groups increased,while PaCO_(2) and respiratory rate decreased,and those in the observation group were better than the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P<0.05).After treatment,the comfort score in the two groups increased,APACHEⅡscore decreased,and those in the observation group were better than the control group(P<0.05).Conclusion NIPPV can effectively improve the respiratory function of patients with ARDS,shorten the hospital stay and reduce the incidence of complications,which is worthy of clinical promotion and application.
