妊娠晚期GBS定植对孕妇阴道微生态及妊娠结局的影响

Effect of GBS colonization on vaginal microecology and pregnancy outcome of pregnant women in late pregnancy

目的探讨妊娠晚期B族链球菌(GBS)定植对孕妇阴道微生态及妊娠结局的影响。方法选取2014年12月至2018年12月于我院产科接受产前检查的200例妊娠晚期GBS定植孕妇(观察组)与200例妊娠晚期健康孕妇(对照组)作为研究对象。比较两组的阴道微生态指标、分娩方式、不良妊娠结局及新生儿Apgar评分。结果观察组的阴道pH、阴道清洁度评分、阴道微生态失衡发生率均高于对照组(P<0.05)。观察组的阴道自然分娩率、新生儿出生后1、5 min的Apgar评分均低于对照组(P<0.05);观察组的剖宫产率、不良妊娠结局发生率均高于对照组(P<0.05)。妊娠晚期GBS定植与阴道pH、阴道清洁度评分呈正相关,与新生儿Apgar评分呈负相关(P<0.05)。结论妊娠晚期GBS定植会对孕妇阴道微生态环境、妊娠结局产生不良影响。

Objective To investigate the effect of group B streptococcus(GBS)colonization on vaginal microecology and pregnancy outcome of pregnant women in late pregnancy.Methods A total of 200 pregnant women with GBS colonization in the late pregnancy(observation group)and 200 healthy pregnant women in the late pregnancy(control group)who received prenatal examination in the obstetrics department of our hospital from December 2014 to December 2018 were selected as the research subjects.The vaginal microecological index,delivery mode,adverse pregnancy outcome and neonatal Apgar score were compared between the two groups.Results The vaginal pH,vaginal cleanliness score and the incidence of vaginal microecological imbalance in the observation group were higher than those in the control group(P<0.05).The vaginal natural delivery rate and neonatal Apgar score at 1,5 min after birth in the observation group were lower than those in the control group(P<0.05);the cesarean section rate and incidences of adverse pregnancy outcome in the observation group were higher than those in the control group(P<0.05).GBS colonization in late pregnancy were positively correlated with vaginal pH and vaginal cleanliness score,and negatively correlated with neonatal Apgar score(P<0.05).Conclusion GBS colonization in late pregnancy will have adverse effects on vaginal microecology and pregnancy outcome.
