缩宫素联合卡前列甲酯栓治疗子宫收缩乏力性产后出血的临床效果

Clinical effect of oxytocin combined with methyl carboprost suppository in the treatment of postpartum hemorrhage due to uterine atony

目的分析缩宫素联合卡前列甲酯栓治疗子宫收缩乏力性产后出血的临床效果。方法回顾性分析2018年11月至2020年8月于我院确诊为子宫收缩乏力性产后出血的96例患者的临床资料,按治疗方式不同将患者分为A组(48例,缩宫素)和B组(48例,缩宫素联合卡前列甲酯栓)。比较两组的治疗效果、产后出血量、子宫复旧情况、凝血功能指标和不良反应发生情况。结果B组的治疗总有效率显著高于A组(P<0.05);B组产后2、24 h的出血量少于A组,产后1、3 d的宫底高度低于A组(P<0.05);治疗后,两组APTT、PT均缩短,D-D水平均下降,FIB水平均升高,且B组显著优于A组(P<0.05);两组的不良反应总发生率无显著差异(P>0.05)。结论缩宫素联合卡前列甲酯栓治疗子宫收缩乏力性产后出血安全有效,能够显著改善患者凝血功能,减少出血量,加快子宫复旧进程。

Objective To analyze the clinical effect of oxytocin combined with methyl carboprost suppository in the treatment of postpartum hemorrhage due to uterine atony.Methods The clinical data of 96 patients diagnosed with postpartum hemorrhage due to uterine atony in our hospital from November 2018 to August 2020 were retrospectively analyzed.The patients were divided into group A(48 cases,oxytocin)and group B(48 cases,oxytocin combined with methyl carboprost suppository)according to different treatment methods.The treatment effect,postpartum blood loss,uterine involution,coagulation function indexes and the incidence of adverse reactions were compared between the two groups.Results The total effective rate of tretment in the group B was significantly higher than that in the group A(P<0.05).The amount of bleeding in the group B at 2 and 24 h after delivery were less than those in the group A,and the height of uterine fundus at 1 and 3 d after delivery were lower than those in the group A(P<0.05).After treatment,APTT,PT in the two groups shortened,D-D level decreased,FIB level increased,and those in the group B were significantly better than the group A(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Oxytocin combined with methyl carboprost suppository in the treatment of postpartum hemorrhage due to uterine atony is safe and effective,which can significantly improve the coagulation function,reduce the amount of bleeding,and accelerate the process of uterine involution of patients.
