评估超声内镜(EUS)对胃黏膜下肿瘤(SMTs)的诊断价值及分析胃SMTs的EUS下特点

To Evaluate the Diagnostic Value of Endoscopic Ultrasound(EUS)for Gastric Submucosal Tumors(SMTs)and Analyze the Characteristics of Gastric SMTs Under EUS

目的:对胃黏膜下肿瘤(SMTs)采用超声内镜(EUS)诊断后的价值及胃SMTs的EUS下特点进行综合分析。方法:将2017年5月~2020年5月本院收治的经超声内镜诊断后行内镜黏膜下剥离术(ESI)切除治疗的胃SMTs患者作为此次研究的主要对象,纳入患者共80例以内镜黏膜下剥离术及术后病理学及免疫组化结果分别作为病变起源和病变类型的金标准,评估超声内镜对胃黏膜下肿瘤的诊断价值及分析胃SMTs的EUS下特点。结果:超声内镜诊断后胃黏膜下肿瘤患者的病变起源及病变类型符合率分别为92.86%和84.62%;经对比组间差异存在统计学意义(P<0.05)。结论:超声内镜对胃黏膜下肿瘤患者的病变起源及病变类型均具有较高的诊断价值,但是对于少见病变仍存在一定的局限性,需要通过切除病变来明确诊断。

Objective:To comprehensively analyze the diagnostic value of gastric submucosal tumors(SMTs)using endoscopic ultrasound(EUS)and the characteristics of gastric SMTs under EUS.Methods:From May 2017 to May 2020,patients with gastric SMTs who underwent endoscopic submucosal dissection(ESI)resection and treated in our hospital were the main subjects of this study.A total of 80 patients were included.The results of endoscopic submucosal dissection and postoperative pathology and immunohistochemistry were used as the gold standards for the origin and type of lesions,respectively,to evaluate the diagnostic value of endoscopic ultrasonography for gastric submucosal tumors and analyze the characteristics of gastric SMTs under EUS.Results:After endoscopic ultrasound diagnosis,the coincidence rates of the lesion origin and lesion type of patients with gastric submucosal tumors were 92.86%and 84.62%respectively;the difference between the groups was statistically significant(P<0.05).Conclusion:Endoscopic ultrasound has high diagnostic value for the origin and type of gastric submucosal tumors.However,there are still some limitations for rare lesions.The diagnosis needs to be confirmed by resection of the lesions.
