动态脑电图在脑梗死继发癫痫患者诊断中的应用价值 被引量:1

Application Value of Ambulatory Electroencephalogram in the Diagnosis of Patients with Epilepsy Secondary to Cerebral Infarction

摘要 目的探讨脑梗死继发癫痫患者诊断过程中采用动态脑电图的应用价值。方法在2018年1月—2020年1月期间便利选择在该院收治的54例脑梗死继发癫痫患者当做观察组,选择同一时间段在该院收治的54例脑梗死无癫痫患者作为对照组,对照组实施动态脑电图诊断,观察组先后采用常规脑电图诊断和动态脑电图诊断,对比两组的动态脑电图诊断结果,并在观察组组内对比常规脑电图与动态脑电图的诊断结果。结果观察组脑电图的异常率为83.33%,痫样放电率为75.93%,均高于对照组的5.56%、3.70%,差异有统计学意义(χ^(2)=66.150、58.772,P<0.05);观察组深部白质区的癫痫样波发生率以及脑叶皮质区的癫痫样波发生率分别为74.07%、55.56%,均高于对照组的11.11%、3.70%,差异有统计学意义(χ^(2)=43.776、34.816,P<0.05);观察组内,动态脑电图诊断的异常率(83.33%)和痫样放电率(75.93%)均高于常规脑电图诊断的异常率(57.41%)和痫样放电率(44.44%),组内差异有统计学意义(χ^(2)=8.704、11.167,P<0.05)。结论脑梗死继发癫痫患者在诊断过程中采用动态脑电图诊断,能够对癫痫发作进行有效预测,还能够对患者病情的变化发展进行了解,从而为临床治疗提供重要的依据,值得推广。 Objective To explore the application value of ambulatory electroencephalogram in the diagnosis of patients with epilepsy secondary to cerebral infarction.Methods From January 2018 to January 2020,54 patients with epilepsy secondary to cerebral infarction admitted to the hospital were convenient selected as the observation group.54 patients with cerebral infarction and no epilepsy admitted to the hospital at the same time period were selected as the control group.The control group was diagnosed with ambulatory EEG.The observation group was diagnosed with conventional EEG and ambulatory EEG to compare the two groups of the diagnostic results of ambulatory EEG,compared with the diagnostic results of conventional EEG and ambulatory EEG in the observation group.Results The abnormal rate of EEG in the observation group was 83.33%,and the epileptiform discharge rate was 75.93%,which were both higher than those of the control group 5.56%and 3.70%.The difference was statistically significant(χ^(2)=66.150,58.772,P<0.05);The incidence of epileptiform waves in the deep white matter area of the group and the incidence of epileptiform waves in the cortical area of the brain were 74.07%and 55.56%,respectively,which were higher than 11.11%and 3.70%in the control group.The difference was statistically significant(χ^(2)=43.776,34.816,P<0.05);in the observation group,the abnormal rate of ambulatory EEG diagnosis(83.33%)and epileptiform discharge rate(75.93%)were higher than abnormal rate of conventional EEG diagnosis(57.41%)and epilepsy discharge rate(44.44%),the difference within the group was statistically significant(χ^(2)=8.704,11.167,P<0.05).Conclusion The use of ambulatory electroencephalogram in the diagnosis process of patients with cerebral infarction and epilepsy can effectively predict epileptic seizures and understand the changes and development of the patient's condition,thereby providing an important basis for clinical treatment and worthy of promotion.
