摘要
目的观察振幅整合脑电图(aEEG)在评估促红细胞生成素(EPO)治疗早产儿脑损伤的应用分析。方法选择2019年1月至2020年6月入住佛山市南海区妇幼保健院NICU病房的早产儿,选择胎龄28~34周、体重<2 kg、生后72 h内的早产儿脑损伤患儿80例,随机分成对照组和治疗组,每组各40例。对照组予综合治疗,治疗组在对照组基础上予EPO按250 U/kg,皮下注射。比较治疗情况及治疗前后监测aEEG评分。结果治疗前两组患儿血细胞计数、转氨酶、血肌酐指标相比,差异无统计学意义(P>0.05)。而治疗后较治疗前血细胞计数、转氨酶、血肌酐指数降低,且治疗组患儿血细胞计数、转氨酶、血肌酐、输血次数均少于对照组,而窒息史、肺表面活性物质应用、近期并发症率则低于对照组。治疗组监测aEEG平均评分中均高于对照组(P<0.05)。结论以aEEG评估EPO治疗早产儿脑损伤的治疗,可以改善患儿的血液指标,减少输血次数、窒息与近期并发症发生率,并降低肺表面活性物质的应用,效果理想。
Objective To observe the application of aEEG in EPO in the treatment of brain injury of premature infants.Methods Premature infants admitted to the neonatal intensive care unit(NICU)ward of Maternal and Child Health Hospital of Nanhai District in Foshan City from January 2019 to June 2020 were selected.A total of 80 premature infants with brain injury with gestational age of 28-34 weeks,weight less than 2 kg and postpartum period less than 72 hours were selected,and they were randomly divided into the control group(n=40)and the treatment group(n=40).The control group was given comprehensive treatment,while the treatment group was given EPO with subcutaneous injection at a dose of 250 U/kg on the basis of the control group.The treatments were compared,and the aEEG scores before and after treatment were monitored.Results Before treatment,there were no statistically significant differences in blood cell count,transaminase and serum creatinine indices between the two groups of children(P>0.05).After treatment,the blood cell count,transaminase and serum creatinine indices were lower than those before treatment.And the blood cell count,transaminase,serum creatinine and blood transfusion times in the treatment group were all lower than those in the control group.While the history of asphyxia,application of pulmonary surfactant and recent complication rate of the treatment group were lower than those in the control group.After treatment,the average scores of aEEG in children of the treatment group were all higher than those control gloup,with statistically significant difference(P<0.05).Conclusion The evaluation of EPO treatment for premature infants with brain injury by aEEG can improve the blood index of premature infants,decrease the times of blood transfusion,the incidences of asphyxia and recent complications,and reduce the application of pulmonary surfactant.Therefore,the therapeutic efficacy is satisfying.
