无保护接生法对低危初产妇自然分娩患者会阴裂伤、性生活质量及疼痛的影响

The effect of unprotected delivery method on the perineal laceration,quality of sexual life and pain of low-risk primiparas with natural childbirth

目的探讨无保护接生法对低危初产妇自然分娩患者会阴裂伤、性生活质量及疼痛的影响。方法选取2018年1月至2019年10月期间我院收治的初产妇200例作为研究资料,按照分娩方式不同分为两组,每组各100例。对照组予以传统的接生技术分娩,观察组予以无保护接生法分娩。观察两组患者会阴撕裂伤情况,测评两组患者的护理满意度,比较两组第二产程时间和产后1 d、2 d时的疼痛(VAS)评分,比较两组产妇产褥期后恢复性生活时间、性唤起时间和每月性生活频率,并进行统计学分析。结果观察组产妇满意度为96.00%,显著高于对照组的70.00%,差异有统计学意义(P<0.05);观察组会阴完整率高于对照组,而会阴侧切率低于对照组,差异有统计学意义(P<0.05);观察组产妇产后1 d、2 d时的VAS评分均明显低于对照组,差异有统计学意义(P<0.05);观察组每月性生活频率高于对照组,而产褥期后恢复性生活时间、性唤起时间短于对照组,差异有统计学意义(P<0.05)。结论自然分娩中实施会阴无保护接生技术,可减少会阴侧切,降低会阴裂伤程度与产后伤口疼痛,能够有效改善性生活质量。

Objective To investigate the effect of unprotected delivery on perineal laceration,quality of sexual life and pain in low-risk primiparas with natural delivery.Methods A total of 200 primiparas admitted to our hospital from January 2018 to October 2019 were selected as the research data.According to the different delivery methods,they were divided into two groups,100 cases in each group.The control group was delivered with traditional delivery technology,and the observation group was delivered with unprotected delivery method.The nursing satisfaction of the two groups of patients was evaluated.The VAS scores at 1 d and 2 d postpartum were compared between the two groups during the second stage of labor.The recovery time of sexual life after the puerperium,sexual arousal time and monthly sexual frequency were compared between the two groups,and statistical analysis was conducted.Results The satisfaction of the observation group was 96.00%.The degree of satisfaction was 70.00%higher than that of the control group,and there was a significant difference(P<0.05);The perineal integrity rate of the observation group was higher than that of the control group,but the perineal lateral resection rate was lower than that of the control group,and the difference was significant(P<0.05).The VAS scores of the observation group were significantly lower than those of the control group at 1 and 2 days postpartum,with significant differences(P<0.05).The sexual frequency of the observation group was higher than that of the control group,while the recovery time of sexual life and sexual arousal time of the observation group were shorter than that of the control group,with significant differences(P<0.05).Conclusion The implementation of unprotected perineal delivery technology in natural childbirth can reduce episiotomy,reduce the degree of perineal laceration and postpartum wound pain,and effectively improve the quality of sexual life.
