不同抗凝方案对子痫前期孕妇妊娠结局影响的对比

Effect Contrast of Different Anti-coagulation Therapies on Pregnancy Outcomes for Preeclampsia Gravida

目的研究不同抗凝方案运用于子痫前期孕妇中的价值。方法选择我院2017年6月至2019年10月纳入的80例子痫前期孕妇,按照随机数字法分成两组,每组各40例。研究组采取阿司匹林+低分子肝素,对照组采取阿司匹林,记录两组产后出血量、水肿消退及自觉症状消退时长,分别在干预前后进行24 h尿蛋白定量、平均动脉压测定,同时观察剖宫产率、胎盘早剥、新生儿窒息及胎儿窘迫的发生率,对比两组干预结果。结果干预前两组的24 h尿蛋白定量、平均动脉压对比无意义(P>0.05),干预后研究组各项指标均低于对照组(P<0.05)。研究组产后出血量、水肿消退及自觉症状消退时长均低于对照组(P<0.05)。研究组剖宫产率、胎盘早剥、新生儿窒息及胎儿窘迫的发生率均低于对照组(P<0.05)。结论阿司匹林+低分子肝素运用于子痫前期孕妇中效果显著,可有效改善24 h尿蛋白量及血压水平,改善妊娠结局,保障母婴安全。

Objective To study the clinical value of different anti-coagulation therapies in preeclampsia gravida.Methods Eighty preeclampsia gravida treated from June 2017 to October 2019 in our hospital were selected and randomly assigned to two groups,40 cases in each group.The study group was treated with aspirin and low molecular weight heparin,and the control group was treated with aspirin.The postpartum blood loss,disappearance time of edema and subjective symptoms as well as 24 h urine protein,mean arterial pressure,and occurrences of cesarean section,placental abruption,neonatal asphyxia and fetal distress were compared.Results Before intervention,24 h urine protein and mean arterial pressure between groups were not significantly different(P>0.05);after intervention,related indicators in the study group were lower than control group(P<0.05);compared with control group,the postpartum blood loss,disappearance time of edema and subjective symptoms in the study group were remarkably reduced(P<0.05);the occurrences of cesarean section,placental abruption,neonatal asphyxia and fetal distress in the study group were lower than control group(P<0.05).Conclusion The combination of aspirin and low molecular heparin is proven to be effective,which can improve 24 h urine protein,blood pressure,pregnancy outcomes,and maternal and child safety.
