Analysis of Intervention Effect on Incidence of Deep Vein Thrombosis of Lower Extremity after Operation

目的探讨基于PDCA循环管理多学科协作干预模式降低骨科大手术患者深静脉血栓发生率的效果,并进一步全院推广。方法采用SPSS20.0分析干预前后深静脉血栓发生率及相关评价指标差异。结果医生、护士血栓相关知识平均得分均有所提升,差异有统计学意义(P<0.001);观察组Caprini表和出血及并发症评估表的落实率有明显上升(P<0.001),Autar评分表的及时落实率由82.5%提升至91.7%,但差异无统计学意义(P>0.05);观察组预防措施落实合格率有所提升,达到98.33%,但差异无统计学意义(P>0.05);深静脉血栓发生率由2.30%降至0.39%,差异有统计学意义(P<0.05)。结论应用PDCA循环管理工具可有效降低骨科大手术患者深静脉血栓发生率,具体措施可以标准化,“无栓病房”可在全院推广。

Objective:To explore the effect of the multidisciplinary cooperative intervention mode based on PDCA cycle management on reducing the incidence of deep vein thrombosis in orthopedic patients undergoing major surgery,and further promote it throughout the hospital.Method:SPSS 20.0 was used to analyze the incidence of deep vein thrombosis(DVT)before and after intervention and the differences in related evaluation indexes.Result:The average score of thrombotic knowledge of doctors and nurses was improved,and the difference was statistically significant(P<0.001).In the observation group,the accuracy of Caprini table and bleeding,and complications evaluation table increased significantly(P<0.001),and the timely accuracy of Autar score table increased from 82.5%to 91.7%,but the difference was not statistically significant(P>0.05).The qualified rate of the implementation of preventive measures in the observation group was improved,reaching 98.33%,but the difference was not statistically significant(P>0.05).The incidence of deep vein thrombosis decreased from 2.30%to 0.39%,with statistical significance(P<0.05).Conclusion:The application of PDCA cycle management tool can effectively reduce the incidence of deep vein thrombosis in orthopedics patients undergoing major surgery,the specific measures can be standardized,and the“thrombol-free ward”can be promoted in the whole hospital.
