益气活血通脉汤对胫骨骨折患者术后凝血指标及DVT发生率的影响

Effects of Yiqi Huoxue Tongmai decoction on postoperative coagulation indexes and DVT incidence in patients with tibial fracture

目的:探讨益气活血通脉汤对胫骨骨折患者术后凝血指标及DVT发生率的影响。方法:选取我院2018-07~2020-04收治的97例胫骨骨折患者,根据随机数表法分为对照组49例,观察组48例。对照组术后接受低分子肝素钠治疗,观察组在此基础上加用益气活血通脉汤。对比两组术后14d临床疗效、凝血功能指标、下肢静脉血栓(DVT)及不良反应。结果:观察组有效率85.42%较对照组65.31%高(P<0.05);术后48h,两组PT均降低,APTT、FIB、DD均升高,但观察组PT、APTT高于对照组,FIB、DD低于对照组(P<0.05);观察组术后7d、14d DVT发生低于对照组(P<0.05);两组不良反应对比,未见显著差异(P>0.05)。结论:胫骨骨折术后应用益气活血通脉汤,能够有效提升疗效,改善凝血功能,降低DVT发生率,且不良反应少,具有一定安全性。

Objective:To investigate the effects of Yiqi Huoxue Tongmai Decoction on postoperative coagulation indexes and the incidence of DVT in patients with tibial fracture.Methods:A total of 97 patients with tibial fracture admitted to our hospital from July 2018 to April 2020 were selected and divided into control group(n=49)and observation group(n=48)according to random number table method.The control group was treated with low molecular weight heparin sodium after surgery,and the observation group was supplemented with Yiqi Huoxue Tongmai Decoction.Clinical efficacy,coagulation function indexes,lower limb venous thrombosis(DVT)and adverse reactions were compared between the two groups 14 days after operation.Results:The effective rate of the observation group was higher than that of the control group(85.42%vs.65.31%),and the difference between the two groups was statistically significant(P<0.05).48 hours after surgery,PT decreased in both groups,and APTT,FIB and DD increased,but P and APTT in observation group were higher than those in control group,and FIB and DD were lower than those in control group(P<0.05).The incidence of DVT in observation group was lower than that in control group at 7 and 14 days after operation(P<0.05).There was no significant difference in ADR between the two groups(P>0.05).Conclusion:Yiqi Huoxue Tongma Decoction after tibial fracture surgery can effectively enhance the efficacy,improve the coagulation function,reduce the incidence of DVT,and have less adverse reactions,with certain safety.
