桃红四物汤与低分子肝素预防髋部手术后下肢深静脉血栓的疗效观察

Effect of Taohong Siwu Decoction and Low Molecular Weight Heparin on the Prevention of Thrombosis after Hip Surgery

目的:探究桃红四物汤与低分子肝素在髋部手术后下肢深静脉血栓预防中的临床效果。方法:选择2018年10月至2020年11月于高安市中医院行髋部手术的患者70例,采用随机数字表法分为两组各35例。对照组经皮下注射低分子肝素钠,研究组于上述治疗基础上联合桃红四物汤。对比两组凝血功能各指标[凝血酶时间(TT)、凝血酶原时间(PT)、活化部分凝血酶时间(APTT)、D-二聚体(D-D)]水平、小腿周径及下肢深静脉血栓(LEDVT)发生率。结果:治疗后,两组TT、PT、APTT水平比较差异无统计学意义(P>0.05),研究组D-D水平低于对照组,差异有统计学意义(P<0.05);治疗前及治疗1 d后,两组患者小腿周径比较,差异有统计学意义(P>0.05),治疗14 d后,研究组小腿周径小于对照组,差异有统计学意义(P<0.05);研究组LEDVT发生率为2.86%,低于对照组的22.86%,差异有统计学意义(P<0.05)。结论:桃红四物汤与低分子肝素能够有效预防髋部手术后LEDVT形成,缓解术后小腿肿胀,值得推广。

Objective:To explore the clinical effects of Taohong Siwu decoction and low molecular weight heparin in the prevention of thrombosis after hip surgery.Methods:Seventy patients who underwent hip surgery in Gaoan City Hospital of Traditional Chinese Medicine from October 2018 to November 2020 were selected and divided into two groups with 35 cases by random number table method.The control group was injected subcutaneously with low molecular weight heparin sodium,and the study group was combined with Taohong Siwu decoction on the basis of the above treatment.Comparing the two groups of blood coagulation indexes[thrombin time(TT),prothrombin time(PT),activated partial thrombin time(APTT),D-dimer(DD)]levels,leg circumference and incidence of lower extremity deep venous thrombosis(LEDVT).Results:After treatment,there was no statistical difference in the levels of TT,PT,and APTT between the two groups(P>0.05).The DD level of the study group was lower than that of the control group,and there was a statistical difference(P<0.05);there was no statistical difference in calf circumference between the two groups(P>0.05).After 14 days of treatment,the calf circumference of the study group was smaller than that of the control group,which was statistically different(P<0.05);the incidence of LEDVT in the study group was 2.86%,which was low Compared with 22.86%of the control group,there was a statistical difference(P<0.05).Conclusion:Taohong Siwu Decoction and low molecular weight heparin can effectively prevent the formation of LEDVT after hip surgery and relieve postoperative calf swelling,which is worthy of promotion.
