基于营养药物合理使用管理评价其对喉癌患者术后的影响

Based on the Rational Use and Management of Nutritional Drugs to Evaluate their Effects on Patients with Laryngeal Cancer after Surgery

目的:探讨分析营养药物合理使用管理对喉癌患者术后的影响。方法:分析赣南医学院第一附属医院2017年6月至2018年12月期间收治的46例喉癌患者,将所有受试者分为观察组与对照组。对照组术前、术后给予常规(经验性)营养支持治疗;观察组术前运用NRS 2002联合PG-SGA给予营养风险筛查,根据营养风险筛查结果给予个体化营养支持治疗方案,并及时评估围手术期营养支持治疗效果。对比两组患者相关营养指标、并发症发生情况、住院时间、住院费用等,探讨分析营养药物合理使用管理对喉癌患者术后的影响。结果:观察组患者术后前白蛋白(PA)、淋巴细胞计数(LY)高于对照组,差异有统计学意义(P<0.05);术后感染发生例数低于对照组,差异有统计学意义(P<0.05);住院时间与住院费用低于对照组,差异有统计学意义(P<0.05)。结论:通过围手术期营养风险筛查与评估、制定个体化营养支持治疗方案、规范营养药物临床使用合理化,可有效改善患者术后营养状况及相关指标、减少并发症、缩短住院时间、降低住院费用。营养药物合理使用规范管理在控制医疗成本与医疗费用增长方面具有借鉴意义。

Objective:To explore and analyze the impact of rational use and management of nutritional drugs in patients with laryngeal cancer after surgery.Methods:46 patients with laryngeal cancer in First Affiliated Hospital of Gannan Medical University from June 2017 to December 2018 were analyzed,and all the subjects were divided into observation group and control group.The control group was given routine(empirical)nutritional support before and after operation;the observation group was given nutritional risk screening by NRS 2002 combined with PG-SGA before operation,and individualized nutritional support treatment plan was given according to the results of nutritional risk screening,and the effect of perioperative nutritional support treatment was evaluated in time.The related nutritional indicators,complications,length of hospital stay and hospitalization expenses of the two groups were compared,and the effects of rational use and management of nutritional drugs on postoperative patients with laryngeal cancer were analyzed.Results:The pre-albumin(PA)and lymphocyte counts(LY)of the observation group were significantly higher than those of the control group,and the difference was statistically significant(P<0.05);the number of postoperative infections in the observation group was lower than that of the control group,the difference was statistically significant(P<0.05);the hospitalization time and hospitalization expenses of patients in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:Through the perioperative nutritional risk screening and assessment,the formulation of individualized nutritional support treatment plans,and the rationalization of the standardized clinical use of nutritional drugs,it can effectively improve the postoperative nutritional status and related indicators of patients,reduce complications,shorten the length of hospitalization,and reduce hospitalization cost.The standardized management of the rational use of nutritional drugs has reference significance in the control of medical costs and the growth of medical expenses.
