彩色多普勒超声心动图在老年冠心病患者中的应用

Application of Color Doppler Echocardiography in Elderly Patients With Coronary Heart Disease

目的探讨老年冠心病患者中彩色多普勒超声心动图的应用。方法回顾性选取2016年3月—2019年3月我院老年冠心病患者200例,均接受彩色多普勒超声心动图(UCG)检查和冠脉造影(CAG)检查,分析UCG检查对老年冠心病的诊断价值,统计分析UCG检查对冠心病病变类型、支数的敏感度。结果 200例患者中,CAG检查阳性155例,阴性45例;UCG检查阳性119例,阴性81例。将金标准设定为CAG检查,UCG检查对老年冠心病的诊断敏感度为67.7%(105/155),特异度为68.9%(31/45),准确度为68.0%(136/200),阳性预测值为88.2%(105/119),阴性预测值为38.3%(31/81)。UCG检查对心绞痛型冠心病的诊断敏感度显著低于心肌梗死型冠心病(P <0.05);UCG检查对心绞痛型冠心病双支病变、三支病变55.6%(20/36)、63.2%(24/38)的诊断敏感度均显著低于心肌梗死型冠心病87.5%(14/16)、96.6%(28/29)(P <0.05)。结论老年冠心病患者中彩色多普勒超声心动图的应用价值高。

Objective To explore the application of color Doppler echocardiography in elderly patients with coronary heart disease.Methods 200 cases of elderly patients with coronary heart disease in our hospital from March 2016 to March 2019 were retrospectively selected,all of them received color Doppler echocardiography(UCG)and coronary angiography(CAG),the diagnostic value of UCG for elderly coronary heart disease was analyzed,the sensitivity of UCG to the type and number of coronary heart disease lesions were statistically analyzed.Results Of the 200 cases of patients,155 cases were CAG positive and 45 cases were negative,119 cases were UCG positive and 81 cases were UCG negative.The gold standard was set as CAG.The sensitivity of UCG was 67.7%(105/155),the specificity was 68.9%(31/45),the accuracy was 68.0%(136/200),the positive predictive value was 88.2%(105/119),the negative predictive value was 38.3%(31/81).The sensitivity of UCG was significantly lower than myocardial infarction(P<0.05).The sensitivity of UCG in the diagnosis of two vessel disease,three vessel disease 55.6%(20/36),63.2%(24/38)were significantly lower than in the diagnosis of myocardial infarction coronary heart disease 87.5%(14/16),96.6%(28/29)(P<0.05).Conclusion The application value of color Doppler echocardiography in elderly patients with coronary heart disease is high.
